Abstract: In contrast to slot-type gaming machines with their preset paylines designating predetermined array positions, the paylines of this novel wagering game are unknown until the indicia in the array have been selected. The indicia are selected from a variety of indicium types, each having a different number of members for identifying adjacent indicia. The randomly selected indicia determine the paylines, if any, in the array. Conditions may be imposed that limit the size and geometry of the paylines in this wagering game.

Abstract: A promotional form is provided which includes a detachable element received within the surrounding margin of a base sheet, the base sheet being folded and sealed around one side of the detachable element to initially conceal a message, question or other indicia hidden until the detachable element is at least partially removed. The promotional form may include a plurality of detachable elements having correlated indicia printed on an exposed side and either on the concealed side of the element or in a portion of the opposing form section in registry with the detachable element. A game or educational piece may be inexpensively provided by correlating different indicia messages corresponding to respective detachable elements, so that in response to an initial query or instruction, a plurality of possible results may be revealed, leading in turn to an outcome revealed by removal of a further detachable element.

Abstract: A mailer type business form intermediate, and method of producing a mailer from the intermediate, effect folding in a novel manner in order to produce a business form that is particularly advantageous for providing a negotiable instrument, such as a bank check or money order. The intermediate has conventional components, but with first, second and third fold lines defining a sheet of paper comprising the intermediate into first through fourth panels. The fourth panel has indicia defining the negotiable instrument and at least one line of weakness separating the negotiable instrument from the rest of the fourth panel. Tacking adhesive patterns may be disposed substantially parallel to the top and bottom edges of the intermediate to hold the first panel second face to the second panel second face adjacent the top edge and second fold line, and to hold the fourth panel second face to the second panel first face adjacent the first and third fold lines, when the intermediate is folded about the fold lines.

Abstract: Improvements to self-replicating duplex forms are disclosed. Generally, a single sheet of paper is divided into original and copy panels by fold line, and carbonless coatings are applied to the panels so that information entered on the two, front and back surfaces of the original panel are reproduced on the two surfaces of the copy panel. An endorsable carbonless CB coating is applied to the original panel, either at the mill or on-press. A carbonless CF coating is applied to the copy panel so that the coated copy panel is substantially the same thickness as the coated original panel. Specific areas for filling out information on the original panel are offset, from front-to-back, and methods of checking this offset are disclosed.

Abstract: A carbonless paper notepad for producing multiple copies of a document created on the notepad. The carbonless paper notepad includes a tablet with a plurality of sheets each having a lower face coated with a duplicating material. A top end of the tablet is coupled to a first end of a board. The board has a spaced apart pair of median folds extending between the sides of the board which divide the board into backer and underback panels. The backer panel is positioned beneath the tablet. The underback panel is inserted between adjacent sheets of the tablet beneath the upper sheet of the tablet.

Abstract: A shipping facilitating business form allows an outgoing address label to be imaged at the same time that a record copy is being imaged, and has both outgoing address label and a tracking label on the same form, and further includes two record copies after the labels are removed. A first paper ply cooperates with a second, composite, ply having a first sheet element and a second release liner underlying the first element, the second ply having a portion extending past the first ply. Pressure sensitive adhesive is provided between the first and second elements, and the extension portion of the second ply comprises tracking labels typically with bar code indicia. Using CF, CB, CF--B and/or self-contained techniques, image transfer is provided between the first ply, to a label portion of the first element, and to the release liner underlying the first element, so that when the label--with variable address indicia imaged on it--is removed both the top ply and second element provide record sheets.

Abstract: A multi-part carbonless business form includes a first upper CB sheet having a first heat and/or pressure sensitive adhesive strip or a lower surface thereof and extending along and adjacent one marginal edge thereof; a second lower CE sheet having a second heat and/or pressure sensitive adhesive strip of substantially identical adhesive composition as the first strip, on an upper surface thereof and extending along and adjacent a corresponding marginal edge in substantial vertical alignment with the first adhesive strip; and at least one intermediate CFB sheet located between the first upper sheet and the second lower sheet and having a first plurality of apertures vertically aligned with the first and second adhesive strips to thereby allow heat and/or pressure sealing engagement between the upper CB sheet and the lower CF sheet through the first plurality of apertures in the intermediate CFB sheet.

Abstract: A multi-ply courier waybill comprises a plurality of data plies connected along et least one marginal edge to an underlying backing ply having a pressure sensitive adhesive applied to the undersurface thereof for attachment to a package to be shipped. The backing ply includes a plurality of die cut labels formed over a portion thereof spaced inwardly from the marginal edges of the backing ply, and the pressure sensitive adhesive is covered by a release liner. Removal of the release liner from the surface of the adhesive will result in removal of the die cut labels from the backing ply. At least some of the data plies and the die cut labels ere provided with common bar code indicia. The labels are applied to satellite packages so that only one waybill can accommodate a number of packages sent to the same addressee.

A form set having two single sheets, each sheet delineated into two panels, making four two-sided copies, two of which have original writing on one surface, is described. A single sheet form, delineated into two panels, making two two-sided copies, each of which has original writing on one surface, is described. A single sheet form, delineated into an original and a copy panel, making a two-sided copy of an original having original writing on both surfaces is described. A form set having a single sheet delineated into an original un-coated panel and a CF-coated copy panel, and employing a separate CB-coated image-transferring sheet is described.

Abstract: A record pad of paper sheets includes a number of stacked sets of sheets. Each set comprises a quadrate top sheet having top and bottom faces and for receiving indicia on its top face and having a first edge; a quadrate record sheet having top and bottom faces and for receiving transferred indicia from the top sheet on its top face, and having a first edge extending generally parallel to and outwardly from the first edge of the top sheet to define a margin; a transfer mechanism (such CF and CB constructions) is disposed between the top and record sheets for transferring indicia from the top sheet to the record sheet; and repositionable adhesive is provided on the top sheet bottom face at the top sheet first edge for releasably connecting the top sheet to the record sheet. The pad also includes mechanical fastener or adhesive for securing the record sheets in the pad of stacked sheets together at the margins.

Abstract: The overcoat emulsion for protection a carbon spot or other image transfer medium against smudging is made from a polyethylene wax emulsion. In addition, a stilt such as wheat starch may be added to the emulsion to improve the pressure absorbing qualities of the overcoat. In addition, a surfactant may be added to the emulsion to improve the wettability characteristics of the emulsion when being applied to a substrate such as a paper ply of a mailer.

Abstract: A two part shipping form has top and bottom plies with a shipping label operatively mounted on the second ply. Image transfer mechanisms, such as CF and CB coatings, are provided on the bottom of the first ply and the top face of the label and the second ply. Beneath the pressure sensitive adhesive of the label is a silicone coating or a transparent release material ply to allow the label to be removed from the second ply. Beneath the silicone coating is a self-contained spot, so that indicia written on the top ply transfers to the top face of the label and to the top face of the second ply beneath the label. The forms are made on presses and by blowing the label, with or without the transparent midply, onto the second ply.

Abstract: A mailer or business form is provided including a coating of a first colorless reactant on one sheet thereof and a transparent patch overlying the coating. The surface of the patch facing the sheet includes a coating of a second colorless reactant such that when the transparent patch is impacted by an imaging force, a visible image is formed which may be viewed.

Abstract: A form set for use with an imprinting indicia with dye-based ink on negotiable instruments. The form set comprises at least one negotiable instrument sheet comprised of an absorbing type paper sheet material for absorbing the dye-based ink, whereby when the form set is impacted by the imprinting mechanism of the apparatus, the dye-based ink permeates through the sheet from the printed front surface to the back surface to provide the indicia in mirror image form on the back. The total character face lateral migration of the ink from any character perimeter surface on either surface of the sheet is no greater than about 0.012 inches.

Abstract: A record book or pad and a method of manufacturing such a book or pad has a stack of sheets in sets A. Each set A consists of a primary sheet P disposed between secondary sheets OS and US. Both faces of the sheet P are provided with a reactive coating 4 of duplicating material and each face of the secondary sheets OS and US which opposes the primary sheet P in the set is provided with reactive coatings 5 of duplicating material. When a marking is applied to the sheet OS for recording a message, pressure from that marking transfers through the sheet OS causing the coatings 4 and 5 between the sheets OS and P to inter-react and form a duplicate marking on a one face of the sheet P. Similarly when a marking is applied to the secondary sheet US, pressure from that marking transfers through the sheet US causing the coatings 4 and 5 between sheet P and US to inter-react and form a duplicate of the marking on the second face of the sheet P.

Abstract: A carbon copy set includes a sheet having an elongated edge. An elongated strip of material having an adhesive coating thereon extending longitudinally of the strip on one side thereof is attached to the sheet at its elongated edge by a first portion of the adhesive coating. The strip is disposed such that an elongated edge of the strip and a second portion of the adhesive coating are positioned beyond the elongated edge of the sheet. The sheet and the elongated strip together present a combined sheet having the second portion of the adhesive coating exposed on one side thereof between the elongated edge of the first sheet and the elongated edge of the strip. The set may also include another sheet having an elongated edge. The combined sheet overlies the last mentioned sheet and is attached thereto by the second portion of the adhesive coating such that the elongated edge of the second sheet and the elongated edge of the strip of the combined sheet coincide.

Abstract: A single sheet of paper is divided into at least original and copy panels by fold line(s). Information entered on the two, front and back surfaces of the original panel are reproduced on the two surfaces of the copy panel by carbonless copy treatments. In one embodiment, the original panel is fully coated or partially coated with carbonless CB treatment, on both surfaces, and the copy panel is fully coated or partially coated with carbonless CF treatment. In a variation of this embodiment, the original panel is not coated, and the copy panel is coated in selected areas with carbonless SC treatment. In a second embodiment, the original panel is not coated, an image-transferring panel is fully coated on both surfaces with carbonless CB treatment, and the copy panel is fully coated with carbonless CF treatment. In a variation of this embodiment, the image-transferring panel is formed of a separate sheet of paper.

Abstract: A guest check business form will properly align with and be read by conventional guest check processing machines. The top part of a first, heaviest ply of the guest check has machine readable indicia (e.g. bar code) printed on it. Below the bar code second and third plies are attached to the first ply adjacent their top edges, with the bottom edges of all of the plies substantially in alignment. The adhesive attaching these plies is staggered so that at no point along the form is there adhesive more than one layer thick. The adhesive may be repositionable adhesive, or permanent adhesive. With permanent adhesive it is desirable to provide fine perforations in the second and third plies just below the adhesive attachment of them to the first ply. The first ply is preferably carbonless CF paper, the second ply carbonless CFB, and the third ply carbonless CB.

Abstract: A credit card transaction form set provides a customer copy form which is attached to any and all carbon sheets in the set in a readily detachable manner so that the complete carbon sheets can be removed from the customer copy if desired. The customer copy overlays a bottom, OCR bond form, and preferably is a CF carbonless sheet. The set may be either three part or four part, with the top form a CB carbonless sheet, and the fourth part--if provided--a CFB carbonless sheet. A S/F carbon sheet disposed between the customer copy and the OCR bond is attached to the customer copy by permanent adhesive adjacent an edge of the customer copy opposite its attachment to the other forms of the set, and the customer copy is connected by perforations to the carbon sheet.

Abstract: A confidential information bearing article comprising a cover sheet adhered to a base sheet is provided which is capable of providing hidden confidential information to a recipient. Through the use of camouflage, obscuring, and opacifying and reflectivity increasing coatings, the paper used for the mailer becomes effectively opaque, and information printed on the inside of the article is substantially unreadable from the outside. The article includes an adhesive for securing the base sheet and cover sheet and perforations along the edges to assist the recipient in opening the article.

Abstract: A method of implementing travel planning and product therefor which utilizes forms having printed indicia providing space for information on transportation, accommodation, car rental and the like and which is foldable to provide a convenient envelope-type construction for the receipt of airline tickets and the like, the forms being designated for office, family and traveler use.

Abstract: A manifold form assembly has a front page and a back page, with each page having image-receiving areas disposed thereon. The front page image-receiving areas are non-overlapping with the back page image-receiving areas. A pressure-sensitive image-transferring system is provided integral with the assembly to transfer pressure-created images formed on the front and back page image-receiving areas. Duplicate front and back pages receive images transferred by the image-transferring system. A stub connects the original pages, the image-transferring system, and the duplicate pages into an integral form assembly.

Abstract: A manifold form assembly has a front page and a back page, with each page having at least one image-receiving area disposed thereon. The original front page is disposed on one side of a first sheet and the original back page is disposed on the other side of the first sheet. The first sheet is movable between a first position in which the original front page is exposed for completion and a second position in which the original back page is exposed for completion. A pressure sensitive image-transferring system is provided integral with the assembly to transfer pressure-created images formed on the front and back page image-receiving areas. Duplicate front and back pages receive images transferred by the image-transferring system. A stub connects the original pages, the image-transferring system, and the duplicate pages into an integral form assembly. In one embodiment, a set of carbons transfer the pressure-created images from the original front sheet to the duplicate front sheets.
