  • Patent number: 5248279
Abstract: Improvements and alternate embodiments for two-sided, self-replicating forms are described. Form sets having two single sheets, each sheet delineated into two panels, making three two-sided copies of a two-sided original are described with fully-coated carbonless coatings and with patterned carbonless coatings. A form set having two single sheets, each sheet delineated into two panels, making four two-sided copies, two of which have original writing on one surface, is described. A single sheet form, delineated into two panels, making two two-sided copies, each of which has original writing on one surface, is described. A single sheet form, delineated into an original and a copy panel, making a two-sided copy of an original having original writing on both surfaces is described. A form set having a single sheet delineated into an original un-coated panel and a CF-coated copy panel, and employing a separate CB-coated image-transferring sheet is described.

  • Patent number: 5205787
Abstract: A form for recording information for preparing and delivering a food-type order, such as an order for pizza and the like food and non-food items, comprises a stack of pressure sensitive copy sheets that are joined together along a common edge. Aligned columns are formed upon the sheets to provde overlapped spaces for receiving written information such as the items ordered, prices, delivery addresses and the like. At least some of the sheets are cut transversely to the common edge to form sets of overlapped, narrow, elongated strips, each having the information spaces. A delivery address space is provided adjacent the common edge and is of a length which is roughly twice the length used for recording address information. A tear line is formed on one of the sheets at the inner end of its address space to form a separable delivery tag.

  • Patent number: 5199923
Abstract: A carbon copy set includes a sheet having an elongated edge. An elongated strip of material having an adhesive coating thereon extending longitudinally of the strip on one side thereof is attached to the sheet at its elongated edge by a first portion of the adhesive coating. The strip is disposed such that an elongated edge of the strip and a second portion of the adhesive coating are positioned beyond the elongated edge of the sheet. The sheet and the elongated strip together present a combined sheet having the second portion of the adhesive coating exposed on one side thereof between the elongated edge of the first sheet and the elongated edge of the strip. The set may also include another sheet having an elongated edge. The combined sheet overlies the last mentioned sheet and is attached thereto by the second portion of the adhesive coating such that the elongated edge of the second sheet and the elongated edge of the strip of the combined sheet coincide.

  • Patent number: 5127879
Abstract: A recordkeeping book has a front cover, a back cover and an accordion-folded strip of paper extending therebetween. The strip is divided longitudinally into a "left" portion, disposed within the covers, and a "right" portion, extending out from the covers. Information entered on the front surface of the left portion is reproduced on the back surface of the right portion when the right portion is folded underneath the left portion. Information entered on the back surface of the left portion is reproduced on the front surface of the right portion when the right portion is folded over the left portion. Means for effecting the reproduction of information entered on both surfaces of the left portion onto both surfaces of the right portion are disclosed and include a "male", or "CB" carbonless copy treatment applied to the front and back surfaces of the left portion, and a "female" or "CF" carbonless copy treatment applied to the front and back surfaces of the right portion.
