    Abstract: The device comprises an endoscope shaft through which extends a working channel ending at an outlet side of the shaft. A guiding element extends through the working channel, said guiding element being preferably guided in longitudinal direction in and along the working channel. The device is set up in such a manner that the distal end section of an instrument extending in longitudinal direction next to the shaft can be moved alongside the shaft beyond the outlet side in order to move the working section of the instrument away from the outlet side so as to lengthen the distal end section of the device. The instrument is coupled to the guiding element in order to guide the working section of the instrument in a transverse direction away from the outlet side in a region opposite the outlet side in order to work in the region with the instrument.

    Abstract: A cryosurgical instrument includes a feed line for conveying fluid into an expansion chamber. The feed line has a capillary line section that terminates in the expansion chamber and forms an aperture for the fluid to undergo the Joule-Thomson effect. The flow cross-section of the feed line decreases in at least one transition section of the feed line in the form of a funnel. Following each transition section there preferably follows a step section, in which latter section the flow cross-section is preferably largely constant. The last step section is preferably formed by the capillary line section. Due to the acceleration of the fluid in the transition sections and the abating of pressure fluctuations in the capillary tube section and, optionally in the additional step sections, the expansion range in the expansion chamber is increased, without impeding the backflow of the expanded gas out of the expansion chamber.

    Abstract: An instrument for electrosurgical treatment including an instrument head which forms a conveying body with a gas-fillable lumen. The lumen is connected to at least one first opening and at least one second opening in the instrument head. A primary stream of gas leaves through the first opening. The second opening is arranged such that a secondary stream exits the second opening next to the primary stream and/or with additional secondary streams out of additional second openings—away from the instrument head, at least partially peripherally surrounding the primary stream. An electrode is located in the flow of gas exiting out of the first opening for igniting a plasma between the first opening and the tissue to be treated. Due to the secondary stream accompanying the primary stream, an ignition of the plasma is successful even if the instrument is at a relatively large distance from the tissue.

    Abstract: A sealing instrument (10) including two branches (15, 16) with sealing jaws (19, 20, 36, 37) acted on by a sealing voltage preferably between 80 and 120 volts. Tissue receptacles (53, 54) are separated from one another by a wall-like extension (24), which at its lower edge carries a cutting electrode (31), are defined between the branches (15, 16). A cutting voltage from 300 to 500 volts is applied between a branch (16) and the cutting electrode. When the branches are closed the grasped tissue is sealed simultaneously between the sealing jaws (19, 36 and 20, 37) and is severed by the cutting electrode (31). Even though the tissue is severed completely; for example after 0.3 to 0.5 seconds and the sealing requires more time, the rolls of tissue grasped in the tissue receptacles (53, 54) act as form-fitting protection of the vessel edges in the tool (12).
