    Abstract: A dental implant drill guide for drilling perpendicular holes of different diameters in a jaw bone for receiving an implant has an elongated guide member with a side for engaging the jaw bone, an opposite side, and a third side also for engaging the jaw bone and an opposite and parallel fourth side. A parallel and spaced apart drill guide holes each of a different diameter extend through the guide member between the first and second sides, and parallel and spaced apart drill guide holes each of a different diameter extend through the guide member between the third and fourth sides. A handle is connected to and extends from the guide member and is of a size to be held be the hand of a practitioner for engaging the guide member against the jaw bone. A second version of the drill guide allows for angled application in posterior edentulous sites.

A bar of the guide has opposite ends and opposite surfaces, one being adapted to engage a jaw bone adjacent the site of a hole to be drilled. A guide bore extends in the bar adjacent one end for guiding the drill. First and second guide posts extend from the opposite surfaces of the bar adjacent the other end. The posts are parallel to the bore and are spaced from the bore by a minimum center-to-center implant distance sufficient to maintain at least 3 mm of jaw bone between adjacent implant receiving holes in the jaw bone.

The arrangement includes jaws, pointed projections or other jaw bone ridge engagement structures which engage the ridge area of the upper or lower jaw bone to fix the arrangement in place. A preferably closed cylindrical guide channel is connected to the engagement structure and extends outwardly along and over the jaw bone ridge. The axis of the channel is parallel to an acceptable axis for the pilot or initial hole into the ridge. The bur can be guided along the guide channel for accurately drilling the hole. Another part of the arrangement is used after the initial hole is drilled to accurately space and orient subsequent implant hole in the jaw bone.

    Abstract: A dental implant hole guide extension is for use with or without an arrangement for drilling an initial hole, the arrangement having tooth engagement jaws for engaging a tooth which is adjacent a site in a patient's mouth for receiving the initial hole, a fixing mechanism connected to the jaws for fixing the jaws to the tooth and a guide member connected to the jaws and adapted to extend over the initial hole, the guide member having a first guide aligned with the initial hole. The extension includes a positioning pin for engagement in the first guide, an arm connected to the pin, a guide member engaging fork connected to the arm on one side of the pin for firmly engaging the guide member to hold the arm to the guide member or to rest at two points on the jaw bone crest if the extension is used without the guide, and a second guide connected to the arm on an opposite side of the pin for guiding a dental bur for drilling the subsequent implant hole.

The arrangement includes a pair of C-shaped jaws which engage lingual and buccal surfaces of a tooth and are held to each other by a fixing mechanism, such as a screw. A guide member extends from one of the jaws and has a semi-cylindrical guide that is parallel to an acceptable axis for the pilot hole. The bur can be guided along the guide for accurately drilling the pilot hole.
