A form presented to a user in a computer application program is identified. A token, which is a magnetic stripe card or a RFID tag is received. The token either has a unique identifier, or information that can be used to generate a unique identifier. The unique identifier is associated with a user. User information is obtained based on the identifier. Elements of user information are matched with elements of requested information about the user requested in the form. The form request for information is completed with the matching elements of user information.
