  • Patent number: 4102489
Abstract: A business reply form to be mailed from a sender to a recipient initially comprises two detachably connected folded sealed sheets; namely, a first instruction sheet which is to be retained by the recipient; and a second questionnaire sheet which is to be detached from the first instruction sheet, filled out by the recipient, folded, and returned by mail to the sender. The second questionnaire sheet includes the name and address of the recipient typed thereon in an address space and the first sheet contains a window in registry with the address space through which the name and address of the recipient appears when the form is initially mailed to the recipient. Thus, one entry of the recipient's name and address on the second sheet by the sender enables the form to be mailed to the recipient and also serves to identify the recipient when the second questionnaire sheet is returned to sender.

  • Patent number: 4074911
Abstract: An elongated one piece paper blank which can be folded into a ticket or coupon usable in a lottery, advertising or promotional contest, game of chance and the like. The blank, when folded and glued, forms a ticket having a front panel of generally rectangular configuration which is connected to a similar rear panel by a fold line at one edge. A smaller intermediate panel is connected by a fold line along one edge to the rear panel. The intermediate panel is adhered to both the front and rear panels to form a closed sheath. A removable rectangular panel is positioned in the sheath between the front and rear panels and extends outwardly of the panels along at least one edge thereof. The removable panel is connected to the sheath only by a connection to the intermediate panel along one of its edges which connection is an easily breakable interrupted cut line.

  • Patent number: 4011985
Abstract: A combined advertising insert and mailing envelope is formed from a single sheet which is folded so as to form a single thickness portion and a double thickness portion. The single thickness portion has a fold line which defines a flap, the fold line being coextensive with the crease lines of the pages of the magazine to position the flap and the balance of the insert, or the advertising page, on opposite sides of the saddle of a magazine. The advertising page is provided with a detachable envelope as well as with a plurality of flaps or sections on the double thickness portion which sections form windows. Each of the sections on one of the panels of the double thickness portion may be lifted to expose information printed on the other of the panels which is in registry with the window. The same insert, in some cases with slight modifications, is also suitable for insertion into perfect or adhesive bound magazines, and side-stitched magazines.

  • Patent number: 4008852
Abstract: A mailing card comprises first and second card sections which are foldable together. The first card section contains an adhesive surface for carrying an object such as a photograph. A removable backing sheet overlies the adhesive surface. The second card section has an opening situated for general alignment with the adhesive surface when the first and second card sections are in a folded condition. A transparent cover extends across the opening. A sealing strip extends along the second card section and is foldable into engagement with the first card section to secure the sections in a folded condition. The first and second card sections are attached along a perforation which provides for optional use of the invention wherein both card sections can be folded together to contain, for mailing, an exposed picture and concealed message, or the first card section can be detached from the second card section and mailed individually as a postcard.
