Abstract: A label and incentive item for collecting data in connection with consumer surveys is provided having a pre-approved incentive card with a base layer for attaching to a product package or release paper. The pre-approved incentive card has indicia printed on its outer side. An outer layer is provided covering at least a portion of the pre-approved incentive card. The outer layer may reproduce indicia on a product package obscured by the label and incentive item. The outer layer may have indicia imprinted on its outer side or inner side, for use as a coupon or other valuable purchasing incentive.
Abstract: A label for affixing to a container and delivering an incentive item is constructed from a first layer, a second layer, and an incentive item having a premium associated therewith. The second layer is attached to the first layer and encapsulates the incentive item. The first and second layers are separable to provide access to the incentive item. The second layer may have indicia thereon associated with the container and/or the incentive item. In another embodiment, the label of the present invention may include an intermediate layer to overlie and conceal the incentive item. The intermediate layer may have indicia thereon associated with the container and the incentive item. The incentive item must be activated before a purchaser may redeem the premium. In the preferred embodiment, the incentive item is activated after the purchaser provides consumer related data to a validating entity.

The outer layer attaches the folded strip to the base layer and encloses it between the base layer and outer layer. The outer layer is designed to be severed along at least one edge so as to allow access to the strip and the pre-approved incentive card. The pre-approved incentive card must be activated before a purchaser may redeem the premium.
Abstract: A label for affixing to a container and delivering a foldable document is constructed from an opaque base layer, a folded strip or document, and a transparent outer layer. The outer layer attaches the folded strip or document to the base layer and encloses it between the base layer and outer layer. The outer layer is designed to be severed along one edge so as to allow access to the folded strip or document. The strip or document has at least one line of perforation for separating it into at least two parts. One part is a product information label which forms an outer panel when the strip or document is folded. Another part is a negotiable instrument which is immediately negotiable by either a customer or retailer. The opaque base layer contains indicia which is similar to the product information label, except for some visually noticeable difference.
