Abstract: Systems and methods are disclosed for creating and updating medical records and documents. A user input is received in a first section on an interface comprising unstructured textual data that is associated with a medical record. The user input received in the first section may be utilized to update the medical record preferably without requiring the user to provide an additional second input in a separate section. At least a portion of the unstructured textual data is mapped to one or more attributes associated with a structured format for the medical record. In response to receiving a single data transfer command, an instruction is generated causing a transfer of the unstructured textual data to a second section on the interface associated with the structured format for the medical record. The mapped data is displayed in the second section in accordance with the structured format.

Abstract: In accordance with an embodiment, a remote logical interface for mining medical data records comprises a data processor for receiving input of a selection of criteria for a medical records query, a database of medical records, responsive to a query, for outputting patient medical record data responsive to the query, and a plurality of operations including a means of combining a first and second query and a means of providing medical data not meeting the first and second query. Such remote logical interfaces may be actuated from a centralized location via an authorized user access portal. The electronic medical records query may utilize a plurality of filters among the following: demographics, vitals, laboratories, ICD codes, CPT codes, prescriptions, alerts, medical history, immunizations and encounters/visits among other filters.
