The insert is then placed within a form for a concrete structure such that the base surface of the Y-shaped insert will be coplanar with a surface of the concrete after the concrete has been poured into the form and set. The base wall of the insert is then removed, thus exposing the internal volume. The distal end of a bolt can be inserted into the insert and fixed to the nut.
