    Abstract: A method of facilitating communications between a patient and a health care provider is provided. The method includes: receiving a patient test result at a mobile computing device; transmitting the patient test result from a wireless interface of the mobile computing device; receiving the patient test result at a host computing system; comparing the received patient test result with a predetermined threshold; and if the patient test result exceeds the predetermined threshold, transmitting an alert to a health care provider.
