    Abstract: A surgical access system comprises a trocar, an insufflating optical obturator slidably insertable into the trocar, and a laparoscope slidably insertable into the obturator. A distal end of the obturator comprises a tip, at least a portion of which comprises a wall with a generally uniform thickness comprising a transparent material. At least one vent hole disposed at the obturator tip is fluidly connected to a gas flow channel defined by an interior surface of the obturator and the laparoscope, which is fluidly connected to an insufflation gas inlet disposed at a proximal end of the trocar. Improved optical characteristics of the trocar system permit precise and accurate visual placement thereof into a body cavity. Accordingly the access system is suitable as a first entry surgical access system. Embodiments of the trocar access are also useful for drug delivery, and/or for fluid and/or tissue aspiration.

According to an aspect, a medicinal fluid delivery device includes a tube that defines an interior space that extends between a first and second openings. The device includes a needle including a proximal end and a distal end that substantially align in a first direction. A portion of the distal end is biased to extend in a second direction different than the first direction. A mechanism is attached to the proximal end and configured to position the distal end between a first position and a second position. In the first position, the portion of the distal end is constrained by the interior wall such that is does not extend in the second direction. In the second position, the distal end is not constrained by the interior wall such that it extends in the second direction.

The locking pins are configured to rest in the at least one set of grooves, such that when the plunger is removed, the locking pins pass within a first shallow section of the groove, pass over the ledge, and continue to pass in the deeper depth section of the groove. When the plunger is depressed in a first partial dispensing movement, the locking pins pass in the deeper depth section of the groove until the plunger depression is temporarily locked when the locking pins reach the ledge, where the dose remaining in the cavity is the specific dosage.
