    Abstract: Methods and apparatus treat a respiratory disorder. For example, a pressure generator supplies a flow of air at positive pressure to a patient's airway through a patient interface. A sensor generates a signal representing respiratory flow rate of the patient. A controller controls the pressure generator to provide to the patient interface a ventilation therapy having a base pressure. The controller computes a measure of ventilation of the patient from the signal. The controller computes a measure of flow limitation from an inspiratory portion of the signal. The controller computes a ratio of the measure of ventilation and an expected normal ventilation. The controller adjusts a set point for the base pressure of the ventilation therapy based on the measure of flow limitation. The adjustment may further depend on a comparison between the ratio and a relative ventilation threshold that increases as the measure of flow limitation increases.

A signal processing unit can receive data from the sensor and can at least one of (i) identify the detected patient anatomy, for example, vocal cords, (ii) determine a distance between the detected patient anatomy and a known point on the endotracheal tube, and (iii) verify a positioning of the endotracheal tube within a tracheal or esophageal lumen of the patient. In some embodiments, the system can include at least one inflatable component that can extend along an outer surface of the endotracheal tube and support the at least one sensor.

Specifically, this disclosure relates to a method and a system configured for use with an oxygen delivery system that includes nasal fitting which does not include nostril prongs. Because the nasal fitting does not include nostril prongs, user comfort is dramatically increased relative to prior designs. That said, because the fitting is free of nostril prongs, changes in pressure associated with the user's breathing register less than in oxygen delivery systems with traditional fittings (i.e., those that include nostril prongs). As such, the method and system of this disclosure is configured to associate relatively small changes in pressure with a breathing cycle of a user, thereby permitting effective and efficient POC operation.

Fraction of oxygen inspired by the patient may be determined based on the average peak inspiratory flow rate and may be adjusted through high-flow oxygen therapy meeting inspiratory demand to prevent entrainment of ambient air or through low-flow oxygen therapy by accounting for entrainment of ambient air based on the average peak inspiratory flow rate to address medical needs of the patient.
