    Abstract: A device is provided for obtaining an image of eyewear from a forced perspective such that measurements of the eyewear can be obtained from the image. The device includes a horizontal alignment indicator and a vertical alignment indicator that each span across two non-coplanar surfaces. When an image is captured from the proper predefined horizontal and vertical positions with respect to the device, the horizontal and vertical indicators will align across the two non-coplanar surfaces. The device can also include a distance indicator. The device can be constructed from a kit that can be easily mailed in a flat envelope. A mobile device can be configured to assist with capturing the measurement image and provide feedback to assist the user to properly position the mobile device to obtain the measurement image.

The device includes a reference probe, a measuring probe for measuring a sagittal depth of a lens relative to the reference probe and a digital display to show the measurement information. The device can further include a refractive index input button that allows the device to measure the optical power of lens composed of materials having different refractive indices. The device can further include a calibration button that is used to set a reference or zero reading by placing the reference probes and measuring probe on a flat surface. A tube level can also be included on the device to assist orienting the device when taking measurements.
