Abstract: A proactive disease state management system including network elements, computer elements and software elements operable to support a plurality of subscriber devices each under a subscription plan in which the subscriber device requests the collection of electronic medical records of a patient for evaluation by a proactive disease state management engine which interrelates patient information, patient medical information and patient laboratory information included in the electronic medical record of a patient to generate engine result data and populate the fields of a request reply to the subscriber device.

Abstract: A window that is installed on a fence or gate is disclosed. The window is installed in an existing fence or gate and allows an animal to look through the fence or gate at people cars or other objects that are on the other side of the fence or gate. The window is fabricated from a single clear material and is printed marked or painted with a border that simulates a frame and or roof structure. Installation of the window requires removing a section of an existing gate or fence and installing the window over the removed section. Since the window is made from a closed surface material sound from the animal such as barking is restricted from transmitting through the window, and also prevents children from sticking their fingers or food through the window.

Abstract: A body fluid collection and assay device that allows for the collection, testing, diagnostics, evaluation, analysis of the fluid within the device. The assaying device includes a handle for easier transportation by one person. A fluid transfer vent is located on a wall within the cup to limit the amount of sample that transfers from the collection portion of the cup to the testing portion of the cup. The assaying device further incorporates an angled base that allows the cup to sit on a table or shelf in a reclined orientation allowing for easier viewing of the assaying strip(s). The collection container incorporates a front viewing window that allows the results of the testing to be viewed through. The collection container device further allows for a screw-down lid that can be placed onto the container to keep the fluid within the cup from spilling.
