Abstract: A healthcare management system is presented where the management system can include a data routing engine configured to route patient data to various authorized providers as a function of one or more trigger rules. The routing engine can operate as a web portal, through which clients of system can define medical form templates, where a template can include one or more triggering rules that cause further action to take place. When a patient interfaces to the web portal, the portal can render a medical form for the patient based on the template. As the patient interacts with the form, including entering their patient data, the routing engine can determine if any triggering criteria have been satisfied. If so, further actions can place including routing the data to authorized entities or providing notifications.
