The cam includes elongated guides (61). A cabinet mounting bracket (72) is coupled to the pivot pin. The hinge is assembled by coupling the cam follower onto the pivot pin prior to placing the combination into the cartridge with the pivot pin square shoulder section received within the cam follower square mounting hole to prevent relative rotation therebetween.
