The tool holder includes: a leading head region having a forward face that defines an aperture for receiving the cutting tool; a trailing shank region with a central longitudinal axis and having an axial forward end and an axial rearward end; and a mediate region intermediate of and contiguous with the leading head region and the trailing shank region. The trailing shank region includes an anti-rotation portion and a mounting retention portion.
