The header panel and the footer panel each flex in an analogous manner with each other between a fold-in orientation and a fold-out orientation and they extend from panel creases that lie between the side panels and the transition panels. The central panel is between the transition panels and separated from them by transition creases. Both the header panel and footer panel have segment creases about which portions of them pivot. A platform extends over the footer panel and simultaneously extends underneath the two transition panels and the central panel.
