    Abstract: A bipolar electrosurgical instrument for clamping, grasping, manipulating, and sealing tissue includes first and second shafts each having a jaw member extending from a distal end thereof and a handle disposed at a proximal end thereof. The handle being operable to effect movement of the jaw members relative to one another from a first position wherein the jaw members are disposed in spaced relation relative to one another to a second position wherein the jaw members cooperate to grasp tissue therebetween. The bipolar instrument is connectable to a source of electrical energy having a first electrical potential connected to one of the jaw members and a second electrical potential connected to the other of the jaw members such that the jaw members are capable of selectively conducting energy through tissue held therebetween to effect a seal. Both the first and second electrical potentials are transmitted to the jaw members through the first shaft.

The handle includes a fixed handle and a handle which is movable relative to the fixed handle to effect movement of the jaw members from the first position to the second position for grasping tissue. The jaw members connect to a source of electrosurgical energy such that the opposable sealing surfaces are capable of conducting electrosurgical energy through tissue held therebetween. A stop is included for maintaining a minimum separation distance between opposing sealing surfaces. A ratchet is also included to maintain a closure force in the range of about 7 kg/cm2 to about 13 kg/cm2 between opposing sealing surfaces.

During surgery, the instrument can be used to grasp and clamp vascular tissue and apply bipolar electrosurgical current through the clamped tissue. In one embodiment, the seal surfaces are partially insulated to prevent a short circuit when the instrument jaws are closed together. In another embodiment, the seal surfaces are removably mounted on the jaws.

The sensor detects movement of the electrically conductive element and communicates a signal to the electrosurgical generator relating to the movement of the electrically conductive element. The source of electrosurgical energy supplies electrosurgical energy in response to the signal from the sensor.

    Abstract: A closed-loop control system has a user interface for allowing a user to select at least one pre-surgical parameter. The system also has a sensor module for continually sensing at least one of electrical and physical properties proximate a surgical site and generating at least one signal and a control module for continually receiving the selected parameter from the user interface and the at least one signal from the sensor module. The system processes the signal in accordance with the parameter using at least one of a microprocessor, computer algorithm and a mapping. The control module generates a corresponding control signal relating to the signal from the sensor module, and provides the control signal to the generator. The control module has number of loop control modules with a first loop control module providing a ratio of a real time parameter value to a desired parameter value. The real time parameter value is from the signal from the sensor module.

    Abstract: A valve and irrigator used with a suction tube has an elongate funnel with an axis through an enlarged entrance and an opposed extended exit duct of a flexible elastomer. A flow passage through the elongate funnel connects the entrance to the exit permitting fluid communication. A web across and near the enlarged entrance sealingly separates that from the exit. The web and the elongate funnel are made of silicone elastomer and are adhesively adhered to together. A portal in the web passes therethrough along the axis and is circular in shape to surround in fluid tight engagement the suction tube. A lid, proximally of the web on the enlarged entrance, has an inlet for irrigation and a pass through for the tube; the lid closes the enlarged entrance forming a chamber between the web and the lid. A slit within the web, near the portal and spaced from the axis is in the form of a closed mouth composed of a pair of sealing lips.

    Abstract: A surgical system has a multiple electrode electrosurgical capability in combination with a gas plasma capability for delivery of electrosurgical energy to the tissue or bodily fluids of a patient. The system includes a holder, a source of electrical energy, electrodes connected to the source of electrical energy, one or more passages carried on the holder for transporting ionizable gas, and a source of ionizable gas of a selectable flow rate. The gas passages may include at least one electrode extending toward the operative site. The gas passages may include a part that creates a vortex in the gas flow. The electrodes may be coaxially placed in the gas passages. The system may also be configured such that two electrodes are each shrouded by the ionized gas so that the electrosurgical energy is conducted to the tissue or bodily fluids of the operative site by passing along conductive pathways in the ionized gas from each electrode. There may also be a dielectric barrier between the conductive pathways.

    Abstract: A temporary stent for supporting a region of a vessel in a body comprising a composite stent portion and an actuator portion and methods for the use and manufacture thereof. The composite stent portion is comprised of an elongate perfusable vessel supporting portion adapted to be configurable between a reduced size for placement in the vessel and removal therefrom and an expanded size for structurally supporting the vessel and perfusable end portions connected to and forming ends of the vessel supporting portion and adapted to allow fluid flow therethrough. The composite stent portion is comprised of a plurality of resilient metallic wires coated with a polymeric material to provide stability for both lifting and maintaining a vessel. A method is disclosed for deployment of the composite stent to maintain vessel patency during occurrence of an intimal flap, vasoconstriction or vasospasm.
