Abstract: The present invention includes a multiple part recordation and affidavit form that is used to capture information relating to a prospective registrant or applicant registering to vote and includes a transparent pocket that contains indicia relating to the identity of the registrant or alternatively the status of the voter in his or her application. The form is constructed from a multiple part arrangement of plies that utilizes a carbonless coating enabling the indicia applied on a first layer to transfer to a second or subsequent plies and the face of the envelope.

Abstract: A transaction terminal (10) having a touch screen (20) disposed in a recess (30) in the terminal housing (12) includes an adaptor (25) for facilitating use of the touch screen by persons that are vision-impaired, hearing-impaired or dexterity-challenged. The adaptor (25) is selectively positionable in operative engagement with the touch screen (20). The adaptor (25) includes an insert (50) and a keypad (60, 160, 260). The insert (50) is supported on terminal housing (12) with deck (52) disposed over and in spaced relationship to the touch screen. The deck (52) has at least one discrete opening therethrough. The keypad includes at least one key juxtaposed relative to said least one discrete opening in said deck of the insert. Upon depression of the key, contact is made with a selected portion of the touch screen (20).

Abstract: A point of sale transaction terminal that has a signature capture pad mounted within a platen that is recessed within the top surface of the terminal housing. A screen protector is mounted over the signature pad and is registered thereon so that the protector covers the signature pad. A bezel is mounted upon said platen within the recess of the housing so that the bezel surrounds the periphery of the protector and the capture pad. A latching mechanism is located beneath the bezel which serves to removably secure the bezel to the platen. An access port is provided in said housing that allows restricted access to the latching mechanism for unlatching the bezel.

Abstract: The present invention is a specimen capture and collection form that utilizes a self laminating form assembly having first and second separable parts one of which is used in the collection, capture and retention of a sample of material to be analyzed and the other of which is used for recording the particular transaction for which the sample of specimen was collected. The form assembly is also provided with a unique element which enables the separation of excess sample collection material to be easily and conveniently separated from the remainder of the form assembly so as to not contaminate the specimen as well as prevent the inadvertent removal of the specimen from the form assembly. The construction is also provided with distinct verification areas to confirm authenticity of the form construction.

Abstract: The present invention relates to a business form assembly that is suitable for collecting and transmitting samples and sensitive items. The business form construction of the present invention includes a plurality of envelopes, compartments, capture areas and the like as well as a number of independently operable closure elements to conceal and protect the sample or sensitive item contained within the assembly. The assembly of the present invention also includes one or more removable and repositionable identification pieces such as labels that are provided with indicia that matches one or more of the envelopes, compartments or capture areas of the assembly as well as collection devices to provide integrity verification between the separate components of the assembly.

Abstract: The invention is a transaction terminal having an encryption mode apparatus comprising a secure information entry circuit and a secure mode indicator. The secure information entry circuit is operable to execute an encryption routine including steps that involve application of an encryption algorithm. Further included in the secure mode information entry circuit is cryptographic firmware which adapts the secure information entry circuit so that an encryption mode signal is caused to change state when an encryption routine is called. The indicator of the encryption mode apparatus is made responsive to the encryption mode signal so that the indicator indicates to a user as to whether data entered into the terminal will be encrypted by the terminal.
