    Abstract: Methods and systems for generating a report that ranks patients at risk for readmission to an acute care facility from a post-acute care facility. A system receives, from the post-acute care facility, patient data for a plurality of patients. The system inputs the patient data into a risk machine-learning system that was previously trained using historical patient data and data reflecting any readmissions from one or more post-acute care facilities to one or more acute care facilities. The system determines, by the risk machine-learning system, a risk score for each patient. Each risk score represents risk of a respective patient being readmitted to an acute care facility from the post-acute care facility. The system further generates the report. The report including a list of at least a subset of patients from the plurality of patients ranked from the patient with the highest risk to the lowest risk of readmission.

    Abstract: A method and apparatus for associating scheduled events with other scheduled events and with electronic documents is provided. A user interacts with user interface controls on a computer display to specify a plurality of scheduled events. The user interface controls allow the user to specify that one or more electronic documents are related to each event, and that one or more other events are related to each event. Data representing the events and the associations between the events and electronic documents is stored on a storage device. User interface controls are provided which allow a user to see a list of the events and/or the documents that are related to a particular event. When a user changes the schedule of an event that has related events, a warning indication is generated to remind the user that the schedules of the related events may also have to be changed.

    Abstract: A computer-implemented method and apparatus in a computer system of processing data generated by a first application program in a second application program during runtime. During runtime, the first application program generates a record including a plurality of fields, wherein at least one of the plurality of fields contains data generated by the first application program. Other of the plurality of fields containing descriptive information regarding the data. The record also includes a reference (e.g. a pointer, relative or absolute) to a tag record. The tag record describes the plurality of fields contained in the record. The tag record further recursively references a plurality of tag records each referencing an associated tag record identifying fields in a referred-to tag record. This continues, recursively, until ultimately, a root record is referenced including a self-referential tag identifying the fields in the root record.
