The method of patient management may include receiving appointment data regarding an appointment for a patient with a selected clinician at a predetermined time, inserting an appointment entry on an electronic calendar of the selected clinician, attaching patient data to the appointment entry on the electronic calendar, providing a first electronic link on the appointment entry to medical records of the patient, providing a second electronic link on the appointment entry to a map showing an address of the patient, partially pre-filling a patient visit form using the attached patient data, receiving patient visit data electronically entered by the selected clinician on the patient visit form during the appointment, and storing the patient visit data into a patient database.

    Abstract: A paving asphalt composition for improving the durability and longevity of asphalt road surfaces is disclosed. The paving asphalt composition includes asphalt base stock, heavy residuum of re-refined motor oil, and a chemical modifier. The heavy residuum of re-refined motor oil preferably has an API gravity of at least about 12, a kinematic viscosity greater than about 400 centistokes at 140.degree. F., a flash point greater than about 400.degree. F. and a mass loss less than about 3%. The paving asphalt composition may also include a polymer. A method of forming the paving asphalt composition and a method of paving roads are also disclosed.

    Abstract: A bitumen composition is provided which contains a block copolymer of one or more halogenated vinyl aromatic blocks and one or more conjugated diolefin blocks. The composition does not phase separate upon storage for five days at 160.degree. C. and has excellent ductility and elasticity. The utilization of halogenated block copolymers also results in improved high service temperature viscosity and moduli with excellent resistance to thermal cracking at low service temperatures. The block copolymer is preferably a hydrogenated block copolymer. The bitumen is preferably halogenated.

    Abstract: A bitumen composition comprising an asymmetric, radial polymer containing a plurality of block copolymer arms comprising at least one polymeric block of a monoalkenyl aromatic hydrocarbon and at least one polymeric block of a conjugated diolefin polymer and a plurality of conjugated diolefin polymer arms. The bitumen composition exhibits increased toughness and tenacity as determined by the Benson method and is particularly useful in road paving applications. Best results are achieved when the asymmetric, radial block copolymer is sulfur vulcanized after incorporation into the bitumen composition.

    Abstract: The present invention is an improvement in a process for producing a blend of bitumen and a block copolymer of a vinyl aromatic hydrocarbon and a conjugated diene comprising mixing from 88 to 99.5 weight percent of bitumen with 0.5 to 12 weight percent copolymer at 135.degree. to 250.degree. C., agitating the mixture for at least 30 minutes, adding 0.01 to 2 weight percent sulfur and maintaining the mixture under agitation for at least 30 minutes. The improvement comprises utilizing high shear mixing at a shear rate of at least 10,000 second.sup.-1 to provide said agitation.
