    Abstract: In one embodiment, a medical lead comprises a lead body for conducting electrical pulses and a paddle. The paddle includes an intermediate metal layer, at least an insulative polymer backing layer, and an insulative polymer covering layer. The intermediate metal layer comprises a plurality of features defined by gaps in the metal material in the metal layer such that each feature is electrically isolated from each other feature, wherein each feature includes a respective connector element that is electrically coupled to at least one conductor within the lead body, wherein a portion of the insulative polymer covering layer is exposed above each feature to define a respective electrode for the corresponding feature. Also, the paddle possesses shape memory to cause the paddle to assume a substantially planar orientation when the shape memory is in a relaxed state.

For each of the conductors, an aperture is defined through the second layer of dielectric material to the conductor, thereby defining an electrode. In one preferred embodiment of this aspect, the first layer of insulative material is in the form of a tube and wherein the core is removable from the tube.

The conductive material is then coated with a second layer of dielectric material. Finally, portions of the second layer of dielectric material are removed to form apertures to the conductive material, thereby forming electrodes.

In addition, a structurally unitary cross-head body has a front aperture and a protruding rim about this front aperture. The funnel shaped tip piece protrudes through the front aperture. Also, a die having a surface that defines an aperture is adapted and positioned to abut the protruding rim and to form a seal against the protruding rim, so that no extrudate may leak out between the surface defining an aperture and the protruding rim.

A set of partially baked crusts are covered with sauce and cheese, to create pizza blanks. Orders for pizza are received, each of which includes a specified set of toppings. In response to each order, the specified set of toppings is placed on one of the pizza blanks. The prepared blanks are placed in a rapid cook oven and permitted to remain for long enough to become cooked pizzas. The cooked pizzas are then removed from the rapid cook oven and delivered to the customers in return for payments.

The spring biases the hub to a raised position in which the disk-receiving surface is above the label plate. Finally, the applicator tool forms a central passage sized for selective placement about the alignment post. In one preferred embodiment, the kit includes a label sheet pack and a software disk stored within the base, and a writing utensil secured to the cover.
