Abstract: The invention provides a store and forward type electronic consult record with sufficient information for a dermatological specialist or consultant to render a diagnosis and to electronically submit that diagnosis back to the referring provider either in real time or at a later time. Accordingly, the invention provides a series of user interfaces or templates for collecting data from referring provider relating to the patient and the patient's condition. The information is then assembled into a dermatological consult record that may be stored and/or transmitted to a remote computer where it may be accessed by a consulting provider. The consulting provider may access the dermatological consult record at any time, review the record, add a diagnosis and/or treatment recommendation to the record, store the record and electronically transmit the record back to the referring provider. The referring provider may then administer treatment to the patient as recommended.
