Abstract: A sophisticated benefits financing and life insurance strategy that provides life insurance benefits for members with no cost to members, provides for funding of the strategy, and potentially building excess funds after payment of benefits and repayment of debt for the benefit of current and future members. A welfare benefits trust is first established by a respective legal entity. A funding amount is then received, the amount of which is calculated based on stochastic actuarial models and modern financial theory to provide purchasing of life insurance and/or other benefits for respective members of the welfare benefits plan, of extension risk coverage, and the development of cash value attributable to each of the plan members as is actuarially determined to be appropriate. Upon the death of a plan member, the welfare benefits trust arranges for payment of the member benefit and repays the funding amount attributable to the deceased member, together with accrued interest.

Abstract: A multiple eligibility medical claims recovery architecture. A system is provided to perform post-processing of existing claims that were incorrectly filed in accordance existing claims reimbursement rules and regulations. The system is operable to further provide filtering, either locally or remotely, of claims submitted by a health care provider to a payor in a multiple-eligibility regime. Still further, the system is configurable to provide automatic filing of the claims to multiple payors on behalf of the health care provider. A system is provided to interact with other (PBM) systems and technology to provide real-time processing of claims submitted to determine if claims are correctly filed, pass those that are and reject those that are not and provide a means by which such rejected claims can be completed and redirected to the appropriate payer for approval and payment.

Abstract: A system for on-line provisioning and payment or reimbursement of durable and disposable medical equipment, home health care and home medical equipment. The system includes a distributed electronic cataloging, order taking, submission, shipping and payment system interacting under direction of an established plan formulary. Eligibility for the equipment and services is established on the front end of order processing. The system allows for differentiation as to which items, products or services are directed to a consumer, and which require a local provider.
