    Abstract: Provided herein are vitrectomy instruments and related systems and methods in which example vitrectomy instruments have multiple rotating cutting edges for severing vitreous fibers. An example vitrectomy instrument may include a handle; an outer tube; and an inner tube configured to be rotated within the outer tube in multiple oscillating rotational cycles. The outer tube may include a port disposed at a distal end thereof. The inner tube may include at least first and second forward cutting edges, so that rotation in a first rotational direction results in both the first and second forward cutting edges cutting vitreous fibers drawn into the port. The inner tube may also include one or more backward cutting edges, so that rotation in a second rotational direction results in one or more backward cutting edges cutting vitreous fibers drawn into the port. Additional forward and/or backward cutting edges may be provided.

In some aspects, a plunger member of a syringe includes a top portion, a bottom portion, and a contact portion. The top and bottom portions are in contact with the contact portion, and the bottom portion has a length greater than or equal to a length of the top portion. The contact portion permits the top portion to move independent of the bottom portion in response to a stress in the contact portion exceeding a critical stress. The stress in the contact portion is based on a first force applied to the top portion and a second force opposite the first force applied to the bottom portion.

    Abstract: Provided herein are systems, methods, and apparatuses that include a vitrectomy tip having port features for improved severing of vitreous fibers in a vitrectomy procedure. The vitrectomy tip may include one or more filaments traversing a port formed in the vitrectomy tip. The one or more filaments separate the port into a plurality of openings. When the vitrectomy tip is ultrasonically vibrated, the filaments impact the vitreous fibers causing them to sever. In this way, the filaments serve as severing elements to separate the vitreous fibers from the eye, thereby providing or improving the ability of the vitrectomy tip to perform the vitrectomy procedure.
