    Abstract: A system, method and computer program product for collecting information are disclosed. In general, information is displayed to a user. The displayed information is based on at least a portion of a plurality of record element constructors of a template. Input for the record element constructors is then received from the user in response to the displayed information. A determination is then made as to whether the received input is valid for the record element constructors. The record elements are subsequently generated based on the record element constructors and the received input. The record elements are then stored in a database.
