The first and second panels each have an internal panel portion including a score line adjacent to an edge of the panel. Each internal panel portion is releasably secured to its panel such that the internal panel portion can be pivoted around its score line relative to its panel. The first panel is foldable over, along the first separation line, to secure the first panel to the second panel. The first and second panels are foldable under, along the second separation line, to secure the first and second panels to the third panel. The first and second internal panel portions are of different sizes so that these panels can be folded out in sequence.

When the tab portion is removed, the wheel portion can be released from the hub portion and at least one arch portion to rotate about the hub portion.

    Abstract: A carrier device for a planar item such as a CD is made from a single, four panel, paperboard blank. A cover sheath is formed by first and second panels of the blank which are secured together so as to define a space therebetween. A carrier member is slidably received in the space for sliding movement between a storage position wherein the carrier member is nested within the sheath and a dispensing position wherein the carrier member extends outwardly of the sheath so that a planar item carried by the carrier member is displayed and can be removed from the device. The carrier member is formed by third and fourth panels of the blank which are secured together by an adhesive. The third panel includes an opening therein for receiving a planar item to be carried and dispensed. The device further comprises cooperating end stops formed on the carrier member and the sheath for limiting the sliding movement of the carrier member.
