The average particle size of the ceragenin particles in the composite is in a range from 5 nm to 20 ?m, 50 nm to 10 ?m, 100 nm to 5 ?m, or 1 ?m to 10 ?m. The composite has a high loading of ceragenin particles (e.g., about 10% to about 25%, by weight). The composite has good polymer stability, the ability to release ceragenins from the ceragenin particles disposed in the composite over a sustained period of time at a characteristic elution rate, and the ability to kill large numbers of bacteria and other susceptible microbes over the sustained period of time.

    Abstract: An intervertebral milling instrument for removing calcified fibrous cartilage (CFC) from adjacent vertebrae during spinal fusion or spacer implantation surgery. The milling instrument includes a grinding wheel carried within a guide housing sized to fit closely into the intervertebral space, and to expose the grinding wheel on opposite sides thereof for controlled removal of CFC layers from adjacent cortical end plates. The grinding wheel is rotatably driven as by attachment to a surgical drill, preferably to include saline supply to and suction removal from the grinding site. The controlled removal of CFC layers is limited to about 100 to about 250 microns, thereby avoiding exposing the cortical end plates without excessive bone removal. The instrument is conveniently provided in a kit having multiple guide housings and associated grinding wheels of different sizes to fit into different intervertebral spaces.

    Abstract: An osseointegrated implant for an amputee includes a surgically implanted post-shaped orthopedic implant rigidly supported by patient bone, wherein the implant protrudes through overlying stump tissue to an external location for suitable attachment to a prosthetic limb or the like. A non-invasive electrical stimulation system provides controlled electrical stimulation at the implant-bone interface for achieving rapid and secure fixation by osseointegration in a significantly reduced rehabilitation time. The electrical stimulation system utilizes at least one and preferably multiple electrodes mounted externally onto the patient in close proximity to the implant-bone interface, with an external portion of the implant providing a second electrode. In one preferred form, the implant comprises a base structure of titanium or titanium alloy or the like, coated with a thin film of a highly conductive substance such as gold for improved electrical conductivity.

    Abstract: A prosthetic joint and related placement method are provided for cemented fixation to a prepared patient bone, with a cement layer and mantle of controlled, predetermined thickness. The prosthetic component such as a tibial or femoral component for a knee prosthesis includes an attachment surface bounded by a flow restrictor wall which cooperates with the attachment surface to define an open-sided cavity for receiving a quantity of bone cement. An intruder tool is provided to form a shallow recess in the prepared patient bone for mating seated reception of the flow restrictor wall on the prosthetic component. In one form, the prosthetic component and the intruder tool may be adjustable to fit patient bones of different size.

The prosthesis comprises a tibial component and a femoral component shaped to accommodate articulatory knee joint motion and each including a base surface adapted for enhanced bone ingrowth attachment to cancellous bone of a resected tibia and femur, respectively. The base surface of each component includes an extended surface area protruding into a region of cancellous bone and subdivided into a plurality of attachment zones coated with a selected porous bone ingrowth material, wherein these attachment zones are bounded by uncoated shallow channels for draining fluid from the attachment interface to the exterior of the prosthesis.

    Abstract: An improved tibial component is provided for a knee prosthesis, wherein the tibial component includes a pair of tibial tray members adapted to accommodate independent longitudinal shifting during patient function while retaining a secure and stable fixation with respect to patient bone. The two tibial tray members comprise medial and lateral members for supporting the medial and laternal condyles of a knee joint. These tray members are interconnected by a slide key arrangement which restrains the tibial tray members against relative movement in the anterior-posterior and medial-lateral directions, while permitting relative shifting in a longitudinal direction corresponding generally with a central axis of the patient's tibia. In the preferred form, the two tibial tray members cooperatively define a common anchoring post and include one or more antirotation fins for mechanically locking with patient bone.
