FIG. 1 is a front perspective view of the patient information holding device, in accordance with my invention;

FIG. 2 is a front elevational view thereof;

FIG. 3 is a top plan view thereof;

FIG. 4 is a rear elevational view thereof; and,

FIG. 5 is a right side elevational view (the left side is a mirror image thereof).

The appearance of the bottom surface forms no part of the claimed design.
