Description

FIG. 1 is a top view of the business form, the bottom view being a mirror image thereof;

FIG. 2 is a front view thereof, the rear of the business form being flat and including no surface ornamentation; and,

FIG. 3 is a right view thereof, the left view being a mirror image.

What is claimed is the ornamental design for a business form, as shown and described.
