The plates are removably held in place on the core by the use of a clamping device, such as a vise-grip, for example. Once the form is filled with concrete and ready for the pressure heading step, the downwardly extending legs of the plates serve as ‘shoehorns’ to guide the pressure header over the top of the liner and prevent damage to it. In addition, the plates will assist in centering the core within the jacket.

The method for producing a terminal post comprising an embedded member at one time uses a cast-on-strap mold for producing a terminal post comprising an embedded member at one time and it comprises the following steps: install the embedded member into the cast-on-strap mold and secure it in position by the fixing bolt, cast and weld the terminal post and the plate pack, and then loosen the fixing bolt to take out the cast and welded terminal post and plate pack.

    Abstract: A fitting for attaching ENT tubing to a concrete form has an inverted cup-like configuration with a bottom opening and an internal cavity within a fitting wall. A flange extends outwardly from the fitting wall around the bottom opening and has fastener-receiving holes for attaching the fitting to a plane surface of a concrete form. A socket extends through the fitting wall into the cavity for receiving an end portion of an ENT tube. The socket has a plurality of circumferentially-spaced resilient fingers that are inclined inwardly toward the socket axis for gripping an ENT tube. A pair of opposite fingers have teeth that are receivable in an annular groove in an end portion of a corrugated ENT tube. The teeth have rounded ends to facilitate ratcheting movement of the ENT tube end portion both into and out of the socket.

Means for holding sections of reinforcing bar can be included and at least one preferred embodiment of the invention includes means for securing the form in its desired location at a construction site. The forms of the current invention are light weight and can be easily assembled at a construction site or at a remote location. Sections of reinforcing bar can be inserted in at least one preferred embodiment of the invention during assembly and the forms can then be folded into a flat configuration for storage until needed. The forms disclosed in the current invention can be reused, but they do not attract insects and therefore do not have to be removed from the pier blocks after the concrete has dried.

The form includes a frame made from metal frame members joined at their ends with the frame members forming the sides of the frame. A reinforcing mat comprising an array of crisscrossed rebars is disposed within the frame and is held in place with a plurality of brackets on the ends of at least some of the rebars and welded to respective frame members. The rebar mat is sized to be positioned in the mid portion of the frame and the brackets on the ends of the rebars are slid outwardly into and attached to the frame members by spot welding. The brackets are then spot welded to the ends of their rebars to form a rigid, lightweight concrete frame.
