Abstract: Various methods and apparatus related to gaming are described. Some embodiments relate to a multi-tiered game involving multiple participants. Some embodiments relate to players establishing gaming elements based on desired criteria. Other embodiments are described.

Abstract: Apparatus and method for secure transactions between gaming machines and portable devices are described. The secure transactions may include a transfer of an amount of an indicia of credit with a cash value from the portable device to the gaming machine or a transfer of an amount of an indicia of credit with a cash value from the gaming machine to the portable device. A logic device, separate from a master gaming controller on the gaming machine and placed in the gaming machine, may be operable to authenticate a portable device, such as a smart card, and authorize transactions involving transfers of indicia of credit between the portable device and the gaming machine.

Abstract: A document carrier sheet is provided with a support sheet having a support surface on which a document is supported. The document carrier sheet is also provided with a transparent sheet attached to the support sheet and permitting the document to be held on the support surface, with an image on the document being covered. The support surface of the support sheet has a different color from that of the document.

Abstract: A promotional form is provided which includes a detachable element received within the surrounding margin of a base sheet, the base sheet being folded and sealed around one side of the detachable element to initially conceal a message, question or other indicia hidden until the detachable element is at least partially removed. The promotional form may include a plurality of detachable elements having correlated indicia printed on an exposed side and either on the concealed side of the element or in a portion of the opposing form section in registry with the detachable element. A game or educational piece may be inexpensively provided by correlating different indicia messages corresponding to respective detachable elements, so that in response to an initial query or instruction, a plurality of possible results may be revealed, leading in turn to an outcome revealed by removal of a further detachable element.

Abstract: A business form with laser compatible punch-out card and the methods of making the combination. The card is permanently adhered to a perforated backer sheet, the periphery of which extends beyond the card and is coated with adhesive for application to the bottom surface of a form containing a hole such that the card is disposed within the hole. The card and attached portion of the backer sheet can then be removed from the card-in-form combination by breaking the perforations. The card is die cut from card stock laminated to the backer layer and is prepared for application to the form by stripping a waste matrix of card material from the laminate through the use of patterned release material.

Abstract: A pocket containing business form, an intermediate for producing the form, and method of producing the business form, provide optimum securement of the panels of the form together with adhesive by providing the adhesive at and along the side edges rather than merely adjacent the side edges. The form may be produced from a web or sheet by moving it in a first direction, applying adhesive patterns (preferably pressure activated cohesive) to the web or sheet along the first direction adjacent but spaced from first and second side edges and at and along a plurality of centerlines between the side edges, and slitting or cutting the web or sheet along the first direction at the adhesive patterns so that adhesive is provided on both sides of each cut or slit, to provide a plurality of forms (e.g. four forms) with form side edges having adhesive at and along the form side edges. If in a web configuration the web is ultimately formed into a sheet, and the sheet is folded (e.g.

Certain variations of the forms can be configured to be extendible to provide additional surface area for the form exceeding standard paper size. The extendible form can be adapted for use with mailing forms such as multi-part collated unit sets. The forms can be useful for providing a Customs Declaration and Dispatch Note having each part thereof printable by a single pass through a non-impact printer.

Abstract: A mailer type business form intermediate, and method of producing a mailer from the intermediate, effect folding in a novel manner in order to produce a business form that is particularly advantageous for providing a negotiable instrument, such as a bank check or money order. The intermediate has conventional components, but with first, second and third fold lines defining a sheet of paper comprising the intermediate into first through fourth panels. The fourth panel has indicia defining the negotiable instrument and at least one line of weakness separating the negotiable instrument from the rest of the fourth panel. Tacking adhesive patterns may be disposed substantially parallel to the top and bottom edges of the intermediate to hold the first panel second face to the second panel second face adjacent the top edge and second fold line, and to hold the fourth panel second face to the second panel first face adjacent the first and third fold lines, when the intermediate is folded about the fold lines.

Abstract: A form for creating a postcard having printing on both sides includes a front sheet and a back sheet, which are laminated using a pressure sensitive adhesive on a back surface of the front sheet. A fold line extends across the front sheet, while a tear line underlying the fold line extends across the back sheet. A gap in the adhesive preferably extends along the fold line. On one side of the fold line opposite to the direction of the gap, the inner surface of the back sheet has a release coating restricting the adhesion of the adhesive layer. After printing on the front surface of the front sheet, the section of the back sheet having this release coating is removed and discarded, and the front sheet is folded along the fold line, thereby providing a document having printing on both sides and a thickness sufficient for a postcard.

Abstract: Discloses sheet of printable media, such as paper, of a special format for printing, preferably by computer, images (such as charts) which are too large to fit on conventional sized sheets of printed paper, and a method for printing such images. The images are printed in sections onto standard sized paper sheets segmented into a retained portion and a removable portion with a glue strip along one side of the retained portion. The sections of the image are printed on the sheet to predetermined margins extending parallel to the sheet edges. The printing may include inconspicuous marks along a registration boundary to facilitate registration of a pair of sheets. The removable portion is separated from the sheet along a segmentation line that coextends with a registration boundary and two sheets with adjacent portions of an image are overlapped until their portions of the image are in register to present a continuous image without gap or overlap.

Abstract: This particular combination of steps in the overall process; the spot coating on a single side of a page, as well as on both sides of the page, in a specific pattern are new and unique. The ability to print multiple orientations on a single side of a page or in duplex fashion is not technology, but is an integral part of the overall process. The folding both horizontally and vertically for the 4-part form, or the “Z” fold for the 3-part form, is not new technology, but is an integral part of the overall process in creating this finished product. The combination of the specific patterns and locations of the spot coatings, the printing in certain areas of the page with certain orientations, and the specific manner in which each single page is folded to become a 3 or 4-part form is unique and patentable.

Abstract: A multi-part, partially pre-printed form useful as an airbill has address information printed thereon by a non-impact printer. Different configurations of the form are described, including panels overlappingly affixed together by an adhesive having removable strips defined by cuts through the top overlapping panels. The center panel is transparent or at least translucent. The address information printed on the outer panels is in a mirror image format relative to the information printed on the transparent panel.

Abstract: A multi-part set providing for so-called “carbonless” copying between parts of the set, comprises a piece of sheet material such as paper with respective portions of at least one face of the sheet being treated to provide for imaging therebetween when such portions lie in face to face relationship, the piece of material being folded to bring the treated portions into face to face relationship and being provided with at least one separation line. Two-part, three-part and four-part sets are disclosed.

Abstract: A form, such as a mailing form, constructed on a single laminated sheet of standard paper size, which is printable by a single pass through a standard non-impact printer, and extendible to provide additional surface area for the form exceeding standard paper size. The extendible form can be adapted for use with mailing forms such as multi-part collated unit sets. The form is useful for providing a Customs Declaration and Dispatch Note having each part printable by a single pass through a non-impact printer.

The form includes a face sheet, a liner ply, an imaging interface interposed between the face sheet and the liner ply, and an adhesive interposed between the face sheet and the liner ply. In accordance with one embodiment of the present invention, a controlled substance dispensing form is provided comprising a face sheet, a liner ply, an imaging interface interposed between the face sheet and the liner ply, and an adhesive interposed between the face sheet and the liner ply to secure the face sheet to the liner ply. The liner ply is configured to define an upper liner ply surface, a lower liner ply surface, and a plurality of releasable liner ply portions. The adhesive is configured to define respective reusable adhesive portions. The face sheet is configured to define an upper face sheet surface and a lower face sheet surface and a plurality of label portions.

Abstract: A multi-part, partially pre-printed form useful as an airbill has address information printed thereon by a non-impact printer. The form includes top and bottom transparent plastic sheets affixed together by an adhesive having three panels defined by perforation cuts through the top sheet and die cuts through the bottom sheet. The adhesive facing side of one of the sheets is preprinted with information appearing on each panel. In addition, a background opaque color is printed over the information on two outer panels, leaving the center panel transparent. The information pre-printed on the opaque panels having is in a mirror image format relative to the information pre-printed on the transparent panel. The address is printed on the outward side of the bottom sheet such that the address printed on the transparent panel is a mirror image relative to the information on outer panels. A four part form is also disclosed.

Abstract: An apparatus and method is provided for imprinting duplicate copies of a credit card receipt using a single-ply roll of paper. The paper can be either coated on a first side with a carbonless coating that contains rupturable microcapsules, each filled with reactive, colorless dyes, and coated on a second side with a resin adapted to react with the colorless dyes, or coated on a single side with self-contained microencapsulated colorless dyes and reactive resins. To include transaction information printed on the credit card receipt as well as information written on the receipt by the customer (such as a signature and an indication of the amount of any gratuity), the transaction information is substantially duplicated in two separate fields each on a separate portion of the credit card receipt. At least part of the two separate portions are then aligned and presented to the customer for completion.

Abstract: A multi-part carbonless business form includes a first upper CB sheet having a first heat and/or pressure sensitive adhesive strip or a lower surface thereof and extending along and adjacent one marginal edge thereof; a second lower CE sheet having a second heat and/or pressure sensitive adhesive strip of substantially identical adhesive composition as the first strip, on an upper surface thereof and extending along and adjacent a corresponding marginal edge in substantial vertical alignment with the first adhesive strip; and at least one intermediate CFB sheet located between the first upper sheet and the second lower sheet and having a first plurality of apertures vertically aligned with the first and second adhesive strips to thereby allow heat and/or pressure sealing engagement between the upper CB sheet and the lower CF sheet through the first plurality of apertures in the intermediate CFB sheet.

Abstract: The invention encompasses multiple-ply carbonless paper rolls having the plies adhesively joined along at least one edge of the roll. Advantageously, the individual plies remain adhered together when the plies are unwound and separated from the main roll but are still relatively easy to separate from each other.

Abstract: Improvements to self-replicating duplex forms are disclosed. Generally, a single sheet of paper is divided into original and copy panels by fold line, and carbonless coatings are applied to the panels so that information entered on the two, front and back surfaces of the original panel are reproduced on the two surfaces of the copy panel.(Group 1) An endorsable carbonless CB coating is applied to the original panel, either at the mill or on-press. A carbonless CF coating is applied to the copy panel so that the coated copy panel is substantially the same thickness as the coated original panel. Specific areas for filling out information on the original panel are offset, from front-to-back, and methods of checking this offset are disclosed. The copy panel is tinted a dissimilar base color from the original panel.(Group 2) The original and copy panels can be formed separately, and joined into a single "virtual" sheet.(Group 3) The original and copy panels can be formed of a single sheet of CF C2S paper stock.

Abstract: A dual impression appointment reminder is made from a two-ply form having a face stock releasably secured by an adhesive onto a release liner of a material such that, upon writing information on the face stock with an impression writing instrument, a copy of that information is simultaneously made on the liner. The face stock includes a removable label at least partially defined by a peripheral cut. The removable label is attachable by adhesive on the underside of it to another surface, such as a calendar. Both the face stock and label have permanent printed information thereon so that upon writing the date and time of an appointment on the label with an impression writing instrument, and upon removal of the label from the liner, two separate reminders are provided with the date and time of the appointment recorded on each of them.

Abstract: In a method for producing composite carbonless copy business forms using non-impact printers, a CFB web having segmented glue lines across the width of both sides of the web. These glue lines adhere the CFB web to adjacent upper and lower CB and CF webs to seal together a business form. The glue lines on both sides of the CFB web are segmented such that the adhesive on the front of the web is laterally staggered from the adhesive on the back of the web. The staggering of the glue line ensures that the adhesive on the front of the web does not adhere to the adhesive on the back when the web is in a roll. Alternatively, the glue line on the CFB web is replaced with a line of holes that align with the glue lines on the CF and CB webs when the webs are registered together.

Abstract: A length of continuous-feed paper, comprising a multiplicity of adjacent paper sheets aligned end-to-end for being fed into a printer. Each of the sheets are defined from adjacent sheets by a cut through the paper, and extending transversely from one side edge of the paper sheet to the other side edge of the paper sheet. First and second continuous paper edge strips are formed on opposite side edges of the paper sheets and extend along the length of the continuous-feed paper for carrying the paper sheets therebetween. The first and second edge strips have sprocket holes therein for permitting the edge strips and the paper sheets carried by the edge strips to be continuously fed through a sprocket-driven printer.

Abstract: A single sheet of paper is delineated into two or more panels, and is coated with carbonless treatment so that information entered on both sides of one of the panels is reproduces onto the other panel(s). The use of carbonless CB and CF, and carbonless SC coatings is disclosed. Methods of making and using the carbonless paper are also disclosed.

Abstract: A single sheet of paper is divided into at least original and copy panels by fold line(s). Information entered on the two, front and back surfaces of the original panel are reproduced on the two surfaces of the copy panel by carbonless copy treatments. In one embodiment, the original panel is fully coated or partially coated with carbonless CB treatment, on both surfaces, and the copy panel is fully coated or partially coated with carbonless CF treatment. In a variation of this embodiment, the original panel is not coated, and the copy panel is coated in selected areas with carbonless SC treatment. In a second embodiment, the original panel is not coated, an image-transferring panel is fully coated on both surfaces with carbonless CB treatment, and the copy panel is fully coated with carbonless CF treatment. In a variation of this embodiment, the image-transferring panel is formed of a separate sheet of paper.

Abstract: A recordkeeping book has a front cover, a back cover and an accordion-folded strip of paper extending therebetween. The strip is divided longitudinally into a "left" portion, disposed within the covers, and a "right" portion, extending out from the covers. Information entered on the front surface of the left portion is reproduced on the back surface of the right portion when the right portion is folded underneath the left portion. Information entered on the back surface of the left portion is reproduced on the front surface of the right portion when the right portion is folded over the left portion. Means for effecting the reproduction of information entered on both surfaces of the left portion onto both surfaces of the right portion are disclosed and include a "male", or "CB" carbonless copy treatment applied to the front and back surfaces of the left portion, and a "female" or "CF" carbonless copy treatment applied to the front and back surfaces of the right portion. Method and apparatus are disclosed.

Abstract: A simple label-type business form is provided that allows a record sheet to be produced at the same time that indicia is applied to the label. The business form includes a label construction having a top face on which indicia is readily applied and a bottom face containing pressure sensitive adhesive; a release sheet having at least one opening; and a self-imaging sheet held in contact with a release sheet on the opposite side of the release sheet from the label construction by the label construction pressure sensitive adhesive. The release sheet preferably has rows and columns of openings, at least three openings in each row and column, and the label construction is a sheet having a number of labels interconnected together, at least one release sheet opening being associated with each of the labels. The self-imaging sheet may have adhesive applied to its bottom face, and a release liner covering that adhesive so that the self-imaging sheet also is a label.
