The first label is fabricated from a first material. A second label is removably affixed to the first side of the substrate adjacent to the first label in a side-by-side relationship. The first labels may be utilized to print desired information thereon and the second labels may fabricated from a transparent, protective material coating which may positioned on the first labels to discourage damage to the first labels from exposure to water or handling when the first labels are affixed to corresponding products.

    Abstract: A prescription paper has reproduction material placed on selected areas of the back side of the paper so that prescription information can be transferred directly onto the patient's records while the prescription writer is writing the prescription. The reproduction material can be a carbonized backing or any other suitable reproduction material. The front side of the prescription paper comprises at least two lines that are separated by a pre-selected distance to match the distance between adjacent lines on the medical record. The prescription writer can align the lines on the prescription paper to the lines on the medical record so that information can be neatly transferred to the space between the lines on the medical record.

    Abstract: A carbonless form is provided that may be folded selectively into a duplicating state or a non-duplicating state. In accordance with one embodiment of the present invention, a carbonless form is provided comprising a form sheet, a transverse fold line, an assigned data entry portion, and an exposed data entry portion. The form sheet defines a front face, a rear face, a top edge, a bottom edge, and a pair of side edges. The transverse fold line extends across the form sheet to define a first panel and a second panel on opposite sides of the transverse fold line. The assigned data entry portion is defined on the rear face of the second panel. The exposed data entry portion is defined on the front face of the first panel. A CB coating is applied to the rear face of the form sheet in the first panel and a CF coating is applied to the rear face of the form sheet in the second panel.

    Abstract: A method for making custom printed notes or note pads. One or more sheet assemblies are provided which have first and second sheets and a pattern of pressure-sensitive adhesive on the rear major surface of each of the sheets that contacts and is releasably adhered to the rear major surface of the other sheet only in nonadhesive areas. Indicia is printed on the front major surfaces of the sheets using a conventional copy machine. The sheets can either be cut along predetermined lines to form the custom printed notes, or the sheets of a plurality of such printed sheet assemblies can be separated, stacked to adhere the patterns of pressure-sensitive adhesive on the rear major surfaces of the sheets to the front major surfaces of the sheets with the edges of the sheets in a predetermined orientation with respect to each other, and the stacked sheets can be cut along predetermined lines to form custom printed note pads.

    Abstract: Improvements to self-replicating duplex forms are disclosed. Generally, a single sheet of paper is divided into original and copy panels by fold line, and carbonless coatings are applied to the panels so that information entered on the two, front and back surfaces of the original panel are reproduced on the two surfaces of the copy panel.(Group 1) An endorsable carbonless CB coating is applied to the original panel, either at the mill or on-press. A carbonless CF coating is applied to the copy panel so that the coated copy panel is substantially the same thickness as the coated original panel. Specific areas for filling out information on the original panel are offset, from front-to-back, and methods of checking this offset are disclosed. The copy panel is tinted a dissimilar base color from the original panel.(Group 2) The original and copy panels can be formed separately, and joined into a single "virtual" sheet.(Group 3) The original and copy panels can be formed of a single sheet of CF C2S paper stock.

    Abstract: A dual impression appointment reminder is made from a two-ply form having a face stock releasably secured by an adhesive onto a release liner of a material such that, upon writing information on the face stock with an impression writing instrument, a copy of that information is simultaneously made on the liner. The face stock includes a removable label at least partially defined by a peripheral cut. The removable label is attachable by adhesive on the underside of it to another surface, such as a calendar. Both the face stock and label have permanent printed information thereon so that upon writing the date and time of an appointment on the label with an impression writing instrument, and upon removal of the label from the liner, two separate reminders are provided with the date and time of the appointment recorded on each of them.

Location ID marks are printed on the card so that an OMR can read the marks.

    Abstract: A simple label-type business form is provided that allows a record sheet to be produced at the same time that indicia is applied to the label. The business form includes a label construction having a top face on which indicia is readily applied and a bottom face containing pressure sensitive adhesive; a release sheet having at least one opening; and a self-imaging sheet held in contact with a release sheet on the opposite side of the release sheet from the label construction by the label construction pressure sensitive adhesive. The release sheet preferably has rows and columns of openings, at least three openings in each row and column, and the label construction is a sheet having a number of labels interconnected together, at least one release sheet opening being associated with each of the labels. The self-imaging sheet may have adhesive applied to its bottom face, and a release liner covering that adhesive so that the self-imaging sheet also is a label.

The article includes an adhesive for securing the base sheet and cover sheet and perforations along the edges to assist the recipient in opening the article.

Duplicate front and back pages receive images transferred by the image-transferring system. A stub connects the original pages, the image-transferring system, and the duplicate pages into an integral form assembly.
