The end portion of the arm is folded to include a first fold and a second fold that combine to reduce a lateral dimension of a portion of the end portion of the arm. The suture extends away from the end portion of the arm. The knot is provided separately from the suture and is secured around the first fold and the second fold to capture both the end portion of the arm and the suture.
