Abstract: Embodiments of the disclosure provide a method of filling out one or more forms by a mobile device. The mobile device detects signals from a beacon and determines proximity to the beacon. The mobile device then receives the forms from a remote server when the mobile device determines it is within a certain distance from the beacon. The mobile device then populates information in the forms and sends the forms to the remote server.

Abstract: A method of managing care requests in a social care network is described. The method includes associating, by a care request processor, a plurality of individuals as members of the social care network. The care request processor receives care request information for an individual member of the social care network and generates and manages tasks based in part on the care request information. Embodiments further include providing contextual advertisements within the network, providing access to personal health information and translating between clinical language and natural language as needed.

Abstract: Systems and methods for supporting the care of a patient are disclosed. Embodiments of the disclosure provide systems and methods for supporting the care of a patient through the use of a patient's support network. The patient can be in any stage of the healthcare system, including being an in-patient at a hospital or other facility or receiving out-patient care. The support network comprises friends, family members and others that have an interest in supporting the patient. The system includes providing at least one member of the support network access to healthcare professionals through a support computing device.

Abstract: A method for attempting to mitigate risk of a health plan member. The method includes: receiving medical data related to the health plan member; computing a first score for the health plan member corresponding to predicated future financial health care costs for the health plan member based on the medical data; computing a second score for the health plan member corresponding to a clinical risk for the health plan member based on the medical data; computing a third score for the health plan member corresponding to a probability of a future acute care event for the health plan member within a threshold amount of time based on the medical data; assigning the health plan member to a risk tier based on the first, second, and third scores; and engaging the health plan member based on the risk tier and one or more engagement factors.

Dentists or health care providers can receive payment from a customer's pre-funded account in exchange for health/dental services provided to a customer, through the use of a health/dental care card. The card is issued to the customer by a health plan organization and is linked to a pre-funded account for that customer. The health care provider is under contract with the organization to offer a predetermined fee structure for covered services.

Abstract: An online health care consumer portal for accessing one or more health-related services by a health care consumer. The consumer portal includes an authentication module for identifying the health care consumer upon receiving an online identification token, and a database for maintaining health care information comprising a plurality of health records. The consumer portal also includes a rules engine module for applying a set of rules to the one or more health records corresponding to the consumer to determine an impairment profile of the health care consumer based on the health records corresponding to the health care consumer and an online user interface for providing access to the health-related services. The online user interface is configurable from a default configuration to a second configuration upon identification by the authentication module, the second configuration adapted to facilitate use of the online user interface according to the impairment profile.

Abstract: A system is provided for computer application code automation comprising a code automation computer server configured for presenting an electronic user interface for receiving user input for generating a Structured Query Language (SQL) query, the user input comprising a plurality of SQL tokens, a processor, associated with the code automation computer server, for receiving the plurality of SQL tokens, the processor retrieving from memory and executing computer executable instructions for at least one of: (a) optimizing performance of the SQL query while receiving the user input, and (b) error checking the SQL query while receiving the user input, wherein the processor is adapted for automatically incorporating the generated SQL query into the computer application code and storing the computer application code in a non-transitory computer readable medium.
