    Abstract: A method for ablating a patient, consisting of ascertaining a CHA2DS2-VASc score for the patient and inserting a probe into the patient, so as to contact a pulmonary vein of the patient. The method further includes applying energy via the probe so as to ablate the pulmonary vein until pulmonary vein isolation (PVI) is achieved. When PVI is achieved and the CHA2DS2-VASc score is less than a preset value, ablation of the pulmonary vein is ceased. When PVI is achieved and the CHA2DS2-VASc score is greater than or equal to the preset value, energy is applied to perform a further ablation.

A balloon insertion mechanism is slidingly located within the chassis channel, and is configured to fixedly accept a balloon sinuplasty mechanism penetrating the tubular probe. A plurality of rollers are disposed within the enclosure and are configured so that on rotation of the enclosure the rollers grip and rotate a guidewire positioned between the rollers, and, absent rotation of the enclosure, release the guidewire and permit distal and proximal translation of the guidewire.

    Abstract: An adapter can include circuitry that can toggle between a mapping state and an ablation state. In the mapping state the circuitry can connect the catheter to a mapping system so that the catheter can measure electrical signals from multiple independent electrodes on an end effector of the catheter. In the ablation state the circuitry can connect the catheter to an ablation generator so that the catheter can apply electrical signals to the electrodes to ablate using IRE and/or RF techniques. The circuitry can short together a group of electrodes in the ablation state and electrically isolate the electrodes in that group from each other when in the mapping state. Using the adapter, the catheter can ablate and map at a treatment site without having to be repositioned between the mapping and ablation steps.
