Abstract: In an exemplary embodiment, a server receives from a communication device (i) an identifier of a patient and (ii) a login credential associated with an individual. The server makes a determination based on the login credential that the individual is authorized to access a patient record of the patient. Subsequent to making the determination, the server prepares an assessment based on the patient record, and provides the assessment to the communication device. The assessment includes a measurement. The server subsequently receives a measurement result from the communication device and updates the patient record based on the measurement result. The server prepares a care plan based on the patient record and provides the care plan to the communication device. The care plan includes a task. The server subsequently receives a task status from the communication device and updates the patient record based on the task status.

Abstract: A home health point-of-care and administration system and method that has at least one mobile device that is adapted to establish bi-directional communication with a server system is provided. Tracking and data communication software are associated with the mobile device in various embodiments. One or more input components of the mobile device allow for input of information from a user (such as a visiting home healthcare staff member) and the mobile device is adapted for receipt of instructional and other information from the server system to build a dynamic visit record. The record provides detailed information related to off-site visits such as tasks performed by the visiting staff member using the mobile device. Start and finish times of the visit may also be recorded. A global positioning system (GPS) application operating in conjunction with the communication device and server system may be employed to monitor the actions of the visiting staff member.

The server system includes modules for staff scheduling, tracking and travel management, visit record and care plan administration, and communications. The server system bidirectionally communicates with a plurality of mobile devices associated with the caregivers to assist in providing home healthcare services, including the ability to better verify visits by caregivers.
