    Abstract: The present invention provides various systems, a kit, and a method for accessing, sampling within, and visualization of areas within the mediastinal cavity for assisting a surgeon in performing a mediastinoscopy procedure. The access system includes one or more preferably toroidal balloons that can be expanded to dilate and protect the inner walls of a bodily conduit. Instruments pass through hollow spaces within the expanded toroidal balloons. The proximally positioned balloons are expanded first and the unexpanded balloons to be positioned distally are passed through them and subsequently expanded. The sampling system includes an instrument with a rounded head having two or more jaws and a slit therein at the distal end of an elongated tubular body. The visualization system includes a 360° camera that can be positioned from proximal to distal a target site and can also do a U-turn about its axis of extension.

    Abstract: The present invention provides an Electro Surgical Generator (ESG) optimized for lung biopsy. The ESG is exclusively battery operated and fits within the handpiece of modern endoscopic electrosurgical/electrocautery instruments, thereby avoiding wires, adapters, and coupling mechanisms. The ESG is adaptable to generate different waveforms that vary with respect to frequency, pulse width, amplitude, etc. through the use of timing circuits and voltage control (i.e. transformers). The ESG is both energy-efficient and safe. A closed loop feedback system featuring a monitor and controller ensure no more power than necessary is provided to achieve a goal current level. Dynamic Power Control (DPC) and Dynamic Temperature Control (DTC) systems vary power to maintain temperature with the lowest possible power. These features prolong battery life and guard against tissue damage.

    Abstract: Methods and devices for obtaining biopsy samples are described. In one embodiment, a capture tube with capture arms slides over a core tube to grasp and/or cut a tissue sample. Forceps with jaws may be used to hold the tissue before the capture tube is used to obtain the tissue sample. In another embodiment, a core tube includes needle channels in which capture needles are advanced to grasp and/or penetrate a tissue sample. In yet another embodiment, a capture tube includes grasping arms which close through slots in a core tube to grasp and/or cut a tissue sample.

    Abstract: An endoscopic biopsy system comprising a means for drawing a sample to be sealed, separated, and/or collected towards an instrument, such as a structure that includes one or more of: an extendable wire, an extendable mast, an extruded tube with at least one opening and a vacuum therein, a pivot point, a hinge, and a mounting block. The system should have a means to transfer energy to a sample after a sample is grasped, such as a conducting wire or an anvil. The system should also have an airtight means to remove a separated sample from a biopsy site for analysis, such as a collection bag or an internal vacuum suction system. The endoscopic biopsy system can be used in a method of obtaining a biopsy from the thoracic cavity that includes a step of insufflating gas to induce pneumothorax and collapse a lung.
