Abstract: A pole base bolt template that provides easy and accurate means to hold pole anchor bolts and a rebar cage in a pole base form while pouring concrete or other filling material into the form. The pole base bolt template comprises a plurality of arms disposed around a central hub, each arm comprising a slotted bolt hole, bolt diameter markings, and a form hook adaptable to secure the template to a form, and each arm being shaped so as to raise the entire template above the top of the form. The template further comprises a plurality of gaps between the arms that allow easy access to the form and the filling material.
