    Abstract: The invention relates to a device incorporating an endocardial acceleration (EA) sensor. A capture test circuit of the device collects a sampled EA signal and extracts a limited series of EA measurements during a duration of a predetermined temporal window opened after delivery of a pacing pulse. An indicator value based on an average of absolute values of successive EA measurements of the series of EA measurements is calculated at an end of the temporal window. The indicator value is compared to a predetermined discrimination threshold to determine the presence or absence of a capture according to whether the indicator value lies above or below the predetermined discrimination threshold. The indicator value is very robust to noise and particularly efficient in terms of computing, which reduces, in large proportions, consumption of the digital processor and thus of the capsule.

The lead also includes an active part having a plurality of microcable conductors, the proximal ends being coupled to the distributor housing, the distal ends being free. Each microcable has a diameter of at most equal to 2 French. Each microcable includes at least one denuded area in the insulating coating forming a stimulation electrode adapted to contact or penetrate an epicardium wall. Each microcable also includes a transverse elongated member extending at an angle relative to the main direction of the microcable for penetrating into the epicardial wall.

The number of VNS pulses of each burst is thus randomly varying, and thus the VNS stimulation energy of this burst and the interval between successive pulses, which counteract the appearance of a physiological compensation loop.

    Abstract: A method for heart treatment includes analyzing the cardiac rhythm. The method further includes utilizing a generator to produce discharges (Si) of N pulses (I) of VNS stimulation in succession. The discharge may be synchronized to a detected R ventricular depolarization wave of each cardiac cycle. The method further includes controlling a stochastic modulation of the discharges control of the delivery of each VNS pulse of each discharge by selective inhibition or not of the generation of these VNS pulses. The number of pulses of each discharge thus randomly varies, and thus varies the VNS stimulation energy of this discharge, which is artificially induced, cycle-to-cycle variability in the RR interval. This stochastic therapy is applied if the spontaneous heart rate variability falls below a minimum level.

The islet is mechanically supported by the insulating and conductive outer layers.
