  • Patent number: 7392952
Abstract: A gift card system includes a gift card imprinted with indicia identifying the issuer, an adhesively coated security label permanently adhered to the card and imprinted on one surface with the value of the gift, and a gift record for recording information concerning the gift including the date of issuance and the value thereof. A recording sheet providing the gift record and the opposite surface of the substrate and the face of the recording sheet have cooperating interactive coatings thereon to imprint the recording sheet. A second security label is substantially permanently adhered on the first security label and imprinted with a reduced value to reflect a partial charge against the value on the first mentioned security label.

  • Publication number: 20070219826
Abstract: A donor processing and information management system disclosed herein allows donor processing centers to facilitate their operations and decrease the use of paperwork and forms by using electronic workstations to enter donor and donation information. Embodiments of the system also facilitate operations of the donor processing centers by automating the management of donors, thus reducing the number of staff necessary to run the center. Donors and donor center staff use workstations to enter biographical data, answer questions, and edit information entered into a database in order to reduce human error and increase efficiency. Thus, embodiments of the system may completely eliminate the use of burdensome forms and automate the entire blood donation process for donor processing centers, whether operating with in-line, real time hardware interfaces at a fixed location, or at mobile locations, where data is collected onsite and transmitted to one or more offsite databases.

  • Patent number: 6889230
Abstract: A computer system and method for generating web pages for displaying formatted customer satisfaction display information and/or presentations on a user computer which may be located remotely and connected to the system by a computer network. The formatted customer satisfaction survey information enables a product or service provider to evaluate the quality of goods and/or services received based on database data obtained by performing surveys of customers, employees, and/or staff. This information can then be compared to normative information, national standards, or other available data. The system utilizes HTML templates, applets, and web servers to generate the web pages. The HTML template process also allows for generation of the HTML templates by programmers. The system and method can be utilized in a quality improvement program.

  • Patent number: 5002105
Abstract: A process is described for making a laminated wood product having improved strength and dimensional stability (i.e., less warping caused by temperature and/or moisture change in drying, transportation and in use). Adjacent surfaces of lumber or veneer laminae cut from a single piece of wood are arranged in units of three laminae where the center lamina is twice the thickness of each outside lamina and in such a way that the outer laminae are rotated 180 degrees about their longitudinal axes to provide mirror images of the surfaces of the center lamina. These trios are arranged in a symmetrical manner about the central plane of the laminate and then bonded together. The minimum number of laminae is three, but with increments of three additional laminae the mirror-laminate can be built up to any desirable thickness. The mirror-laminate may be used in rough form or surfaced to specific finished dimensions.

  • Patent number: 4948173
Abstract: A record keeping assembly has a binder with locating elements spaced along the length of the outer side edge of a panel and a shingled assembly of overlapping elongated sheets with apertures along the one side thereof through which said locating elements extend. Under the shingled assembly is a flexible peel strip which has one end secured to the panel adjacent the outer side and which extends along the locating elements across the panel and projects outwardly therefrom. This peel strip is gripped by its projecting end and lifted upwardly to peel the portion of the shingled assembly overlying the strip from the locating elements.

  • Patent number: 4940257
Abstract: A one write bookkeeping assembly has a folded binder with a pair of hinged panels, locating elements spaced along the length of the outer side edge of one panel, and a pocket opening at the fold line therebetween. A removable insert is seated in the binder and has a multiplicity of spaced rings and a multiplicity of record keeping sheets with perforations along one side and through which the rings extend. A pair of mounting panels are supported on the rings, one extending into the pocket and the other extending over the one panel and having apertures adjacent its outer edge through which the locating elements extend. A shingled assembly of overlapping elongated forms is disposed over the record keeping sheet and the forms have apertures along the one side thereof through which said locating elements extend.

  • Patent number: 4465306
Abstract: A one-write bookkeeping and check writing system has a bank of shingled checks with voucher stubs retentively held by a peg board in overlying registration with a journal sheet. An employee record card for the entry of employee earnings data is folded about one of two fold lines and removably interposed between the bank of checks and the journal sheet with the voucher stubs in registration with the journal sheet. The back side of the record card has a transfer coating to transfer data entered on the voucher stub to the underlying journal sheet. The folded under portion of the record card provides a barrier to the transfer of data other than the data entered on the voucher stub.
