    Abstract: Systems and methods are disclosed for documenting a narrative patient visit note. A user input is received and is associated with narrative phrases which are converted to narrative sentences. These sentences, constructed by the system, are then arranged to form a narrative patient visit note. The narrative patient visit note is constructed in such a manner as to be exportable to numerous electronic medical record systems.
