A healthcare data system collects data from a patient using an interactive voice response system (IVR). The IVR periodically contacts the patient with questions as to severity of symptoms. The severity data is processed along with prior collected data to report treatment progress to the patient, to a provider, to a provider organization and to a payer. Symptom weights are entered by a provider and used in measuring progress. Other methods to collecting severity data may be used, such as email and text messaging.
