    Abstract: A barbed suture including a body with barbs on the periphery, a pointed end, and an anchor at one end to resist movement of the suture in the direction of the pointed end is provided. One or more limbs on the anchor may be provided, which may be arcuate, of varying lengths, and of varying distribution about the periphery of the suture body. Other anchor designs are provided. Methods of placing single-directional and bi-directional barbed sutures to approximate the tissue on each side of a wound and to position and support tissue in the absence of a wound, as in cosmetic surgery, are provided, and may include terminal J-stitches or S-stitches. Methods of placement may be made with sharp, pointed ends, which may be needles, or insertion devices. Sinusoidal patterns of sutures that have amplitudes generally perpendicular to the resultant holding force of the suture are provided.

    Abstract: Disclosed is an infinitely adjustable depth stop for a laparascopic instrument having a shaft, the depth stop including a first component having a first annular space adapted to allow the shaft to be fitted therethrough. The first annular space has a reducible diameter and an interference surface against which the shaft may be fitted. Upon reduction of the reducible diameter, the interference surface frictionally engages the shaft to arrest relative movement of the depth stop along the shaft. The frictional engagement may be along a curve, a spiral curve, or an area. Means are also provided to reopen the reducible diameter to release the depth stop.

The device may be placed through a method such as percutaneous endoscopic gastrostomy (PEG). The device includes a gastrostomy style structure that provides fluid communication with the external environment of the patient with the internal contents of the stomach. The device is produced with a series of slotted collection channels that preferentially, when connected to a source of negative relative pressure, directs a significant volume of the high calorie fluidic components of the chyme out of the digestive tract, preventing exposure to the absorptive tissues of the digestive tract when simple sugars and carbohydrates are consumed.

    Abstract: A surgical instrument for use during a surgical procedure includes an instrument body, an ultrasonic transducer assembly extending along a longitudinal axis, a power cord, and a transducer slip joint. The ultrasonic transducer assembly is rotatably mounted within the instrument body about the longitudinal axis and defines a first outer profile. The power cord projects from the instrument body to provide electrical power to the ultrasonic transducer assembly for operating an acoustic waveguide. The transducer slip joint is positioned between the power cord and the ultrasonic transducer assembly and electrically and mechanically connects the power cord to the ultrasonic transducer assembly. The ultrasonic transducer assembly selectively rotates relative to the power cord for inhibiting the power cord from winding upon rotation of the ultrasonic transducer assembly. The transducer slip joint also defines a second outer profile that fits within the first outer profile of the ultrasonic transducer assembly.

In one embodiment, the cartridge includes a cartridge body having an upper end and a lower end insertable into the cartridge opening. The surgical fasteners are arrayed in a stack that extends between the upper and lower ends of the cartridge body.

The elongated shafts of the second pins have outer diameters that are smaller than the diameters of the second pin apertures for allowing the elongated shafts of the second pins to slide within the second pin apertures.

An actuator activates the firing system to commence a firing cycle. During the firing cycle, the firing system drives the insertion tool toward the distal end of the elongated shaft. The elongated conduit has a top wall with a top notch formed therein. The stripper ramp slides through the top notch for guiding movement of the insertion tool as the firing system drives the insertion tool toward the distal end of the elongated shaft.

    Abstract: A system for deploying a suture implant includes a delivery device and a suture implant coupled with the distal end of the delivery device. The suture implant includes a suture and a slip knot that defines a suture loop, the slip knot defining a dynamic end of the suture loop that is located opposite a closed end of the suture loop. The suture implant includes a tissue anchor having first and second openings, whereby the closed end of the suture loop passes through the first and second openings for securing the tissue anchor to the suture loop. A tensioner is secured to the first end of the suture, and a pledget, located between the tensioner and the tissue anchor, is secured to the second end of the suture. Pulling the tensioner away from the tissue anchor slides the pledget and the slip knot toward the tissue anchor for shortening the length of the suture loop and reducing the distance between the pledget and the tissue anchor.

The proximal shaft section has a proximal outer tube and a proximal inner member, and the distal shaft section includes a distal outer tube and a distal inner member. An incompressible member interconnects the proximal and distal inner members to form an inner shaft member having a fixed length. Distal movement of the proximal outer tube relative to the proximal inner member articulates the distal shaft section relative to the proximal shaft section via the articulating joint. As the articulating shaft moves between a straight configuration and an articulated configuration the distance between a distal-most end of the distal outer tube and the handle changes while the length of the inner shaft member remains fixed.
