    Abstract: In non-limiting examples of the present disclosure, systems, methods and devices for assisting with generating a health score for a patient are provided. An ideal patient profile may be determined from electronic medical record and consumer data received for each of a plurality of patient profiles. Consumer data and electronic medical record data for a patient may be received. A health score for the patient may be determined based on the patient's consumer data and electronic medical record and analysis comprising calculating a sum of patient values for a plurality of factors, wherein the health score is relative to the determined ideal patient profile. One or more modifiable risk factors affecting the patient's health score may be identified based on the consumer and electronic medical record and feedback related to the one or more modifiable risk factors may be provided to the patient.
