Abstract: A method, system, and non-transitory computer readable medium for surveying patients and others in a healthcare facility with real-time feedback, response, analysis, and reporting are provided. The method includes receiving by an interactive device inputs from a user in response to questions in a survey; upon completion of the survey, submitting the completed survey to a survey processing system; analyzing the completed survey by the survey processing system; processing one or more alerts based on the inputs received in response to those questions for which an alert trigger has been met, wherein an alert status is identified as an active alert; receiving and documenting details for each resolved alert, wherein the alert status is changed to a resolved alert; and outputting the completed survey to a display device, wherein the output includes the response to each question and details for each active and resolved alert.
