Abstract: A pocket containing business form, an intermediate for producing the form, and method of producing the business form, provide optimum securement of the panels of the form together with adhesive by providing the adhesive at and along the side edges rather than merely adjacent the side edges. The form may be produced from a web or sheet by moving it in a first direction, applying adhesive patterns (preferably pressure activated cohesive) to the web or sheet along the first direction adjacent but spaced from first and second side edges and at and along a plurality of centerlines between the side edges, and slitting or cutting the web or sheet along the first direction at the adhesive patterns so that adhesive is provided on both sides of each cut or slit, to provide a plurality of forms (e.g. four forms) with form side edges having adhesive at and along the form side edges. If in a web configuration the web is ultimately formed into a sheet, and the sheet is folded (e.g.

Abstract: A multi-ply business form, comprising a chain of custody form having a first section for recording information required by federal drug testing regulations, and a second section containing one or more adhesive labels for labeling containers of body fluid specimens to be tested, and, an airbill form detachably secured to the chain of custody form, the airbill form containing a first part for recording information about sender and recipient of the container, and a second part containing an adhesive label containing information about sender and recipient, the adhesive label arranged to be secured to the container.

Abstract: Radio frequency identification elements, including linerless or lined labels, are produced with enhanced effectiveness. The antenna is printed with conductive ink or toner, and cross-over may be provided by spot printing a non-conductive material over a portion of the antenna and then printing a conductive cross-over element on the non-conductive material. Typically the antenna is printed on one face of a web, and the radio frequency identification chip is moved into contact with the opposite face of the web so that the contacts penetrate the web and engage the antenna so that the web material acts as a dielectric. Alternatively part of the antenna on the first face may be over-printed with a non-conductive material and the chip attached to the first face passing through openings in, or penetrating, the non-conductive material to engage the antenna.

Abstract: A method of packaging and displaying a relative flat product, and intermediate and final package produced, allow a desirably thick package to be produced yet the package can be shipped in a low volume configuration to save shipping costs, and can be expanded at or adjacent the display site in a simple yet effective manner. The package is particularly suitable for packaging a prepaid phone card having machine readable indicia, an opening being provided in the package blank so that the machine readable indicia is visible through the opening so that the phone card can be activated without removing the phone card from the completed package.

Abstract: A multi-part (e.g. three part) business form is particularly suited for use in dispensing sample drugs where it is necessary to have a label to be placed on a sample medication container, a like label to be placed on the patient's chart, and a record sheet, to be placed in the sample medication distribution log. First and second top and intermediate, respectively, label parts each have a face ply having pressure sensitive adhesive on a back surface and in engagement with an adhesive release coated liner ply, and a third, bottom, paper part underlies the first and second parts. Common non-variable indicia is imaged on the first part face ply, the second part face ply, and the third part, and the parts are releasably connected together (e.g. by permanent adhesive and a line of weakness, adjacent the top edges of the parts) so that at least some of the common indicia is in alignment. Carbonless coatings transfer indicia imaged on the first part face ply to the second part face ply and to the third part.

Abstract: A packing list or bill of lading may be easily manufactured from an intermediate comprising a single sheet of paper so that the form can be printed in either cut sheet or continuous format in a simple manner with a minimum amount of adhesive for attaching the form to a package, and with a maximum useful area on the form for informational indicia. The business form is constructed typically by Z or eccentrically Z-folding an intermediate about first and second fold lines to define three plies with aligned first and second lines of weakness formed in the plies substantially parallel to the first and second side edges of the plies and defining first and second detachable side strips preferably between about 1-1.25 inches wide.

Abstract: A shipping facilitating business form allows an outgoing address label to be imaged at the same time that a record copy is being imaged, and has both outgoing address label and a tracking label on the same form, and further includes two record copies after the labels are removed. A first paper ply cooperates with a second, composite, ply having a first sheet element and a second release liner underlying the first element, the second ply having a portion extending past the first ply. Pressure sensitive adhesive is provided between the first and second elements, and the extension portion of the second ply comprises tracking labels typically with bar code indicia. Using CF, CB, CF--B and/or self-contained techniques, image transfer is provided between the first ply, to a label portion of the first element, and to the release liner underlying the first element, so that when the label--with variable address indicia imaged on it--is removed both the top ply and second element provide record sheets.

Abstract: An intermediate for a card holder, the card holder produced from the intermediate, and a method of making the card holder, are provided which are particularly useful for easily and efficiently making and attaching a card holder to a library book, or other object that a card holder is desirably associated with. The card holder is formed from an intermediate having a first (e.g. paper) ply, a second release liner ply, and pressure sensitive adhesive or cohesive holding the plies together. Various die cuts and lines of weakness (such as perf lines) are provided in the plies so as to facilitate easy assembly of the card holder from an integral sheet by removing various die cut strips or sections in sequence to readily fold the intermediate after imaging (for example with a laser printer) and then apply the card holder formed with the pressure sensitive adhesive or cohesive already associated with the intermediate, to a library book or the like.

Abstract: A protected/security document is provided that foils counterfeiting even if a laser photocopy machine (such as a Canon 500 color copier) is utilized. The document has at least three discrete half-tone printed bands disposed on its surface, provided by dots or lines. Each printed band has a different screen density and within each bands the dots or lines comprise a warning word or symbol (e.g. "Void"), or a background. The dots or lines of either the "Void" or background drop out when photocopied, while the dots or lines of the other do not. The dots or lines that do not drop out may be dimensioned so that there are about 24-34 per centimeter, while for those that do drop out there are about 52-64 per centimeter. The bands are typically arranged either linearly or in concentric circles, and interband areas having density gradually transitioning between the densities of adjacent bands are provided.
