    Abstract: A system and methods for automatically processing healthcare data associated with submission and fulfillment of pharmaceutical prescriptions by providers in real time, including claim processing, are enabled by a clinical services platform configured with a review processor and operable with a clinical analytical message (CAM) data file. The system includes the use of first and second databases respectively containing pharmaceutical data and standardized healthcare data. This data is extracted during processing by the system and translated into a common format for storage in a third electronic patient outcome record (EPOR) that is accessible, with full security and patient safety, to authorized providers and patients. Improved computer platforms configured to generate a portable, interoperable patient medical and pharmaceutical record, determine the compatibility of a prescription for a patient, and determine the prescription modification requirements for a patient is presented.

The EDS is to be delivered to the identified patient upon discharge from a treatment facility. The system and process uses first and second data processors operating according to programmed instructions that are linked to corresponding databases and a network of pharmacies to ensure all information in the EDS is delivered to the patient and the medications to be discontinued, including prescription and non-prescription medications, are properly documented in the pharmaceutical records of the patient.
