The form includes a substantially vertical component and a substantially horizontal component. The substantially vertical component extends upwardly from the horizontal component. The at least one bracket includes a vertical bracket section, a horizontal bracket section, and a third bracket section. The vertical bracket section includes an outwardly facing vertical bracket surface and an inwardly facing vertical bracket surface. The horizontal bracket section includes a top facing bracket surface and a bottom facing bracket surface. The vertical bracket section and the horizontal bracket section are fixedly coupled to one another.
